急性心肌梗死IABP植入术后的临床护理

Clinical care experience of acute myocardial infarction after IABP implantation

目的探讨急性心肌梗死主动脉球囊反搏术（IABP）植入术后的临床应用及护理。方法对我科2013年1月-7月收治的22例急性心肌梗死患者应用IABP及综合性治疗，结合专科特点，加强病情观察和生命体征监测，做好管路护理、基础护理和心理护理。结果22例患者应用IABP治疗过程中，16例为支架植入术中和术后应用，拔管后平均15天好转出院；3例于病情稳定后行冠状动脉支架植入术；2例因并发多器官衰竭，最终抢救无效死亡；1例因经济原因而放弃治疗。结论IABP植入术结合有效的护理措施，做好并发症的护理和预防，包括血管并发症、感染和球囊破裂，同时做好病情监测、体位护理和心理护理，能有效改善心肌缺血、缺氧，减轻心脏负担，改善左心室功能，最终提高患者生存率。

Objective To investigate the clinical application of IABP implantation after acute myocardial infarction and nursing. Methods 22 cases of CCU were application of IABP in patients with acute myocardial infarction and com- prehensive treatment, combining with the characteristics of specialized subject, strengthen the observation and monitoring vital signs, complete line nursing, basic nursing and psychological care. Results 22 patients applying IABP treatment, 16 cases of postoperative application of stent implantation and after extubation were an average of 15 days improved, 3 ca- ses were treated in a stable condition after coronary artery stent implantation, 2 cases with complicated with multiple or- gan failure, and eventually died, 1 case because of economic reasons and give up treatment. Conclusion IABP implanta- tion combined with effective nursing measures of nursing and prevention of complications, including vascular complica- tions, infection, and balloon burst, and condition monitoring, nursing and psychological care, posture, can effectively improve myocardial ischemia and hypoxia, reduce heart burden, improve left ventricular function, eventually improve the patients survival rate.
