体表心电图、腔内单极电图与单相动作电位测定心室复极离散度相关分析 被引量:2

Correlations among ventricular repolarization dispersions measured from surface electrocardiogram unipolar endocardial electrogram and monophasic action potential recordings

摘要 目的探讨心室复极离散度测定方法的可靠性。方法对19例无器质性心脏病者 ,应用左、右心室内膜单相动作电位 (MAP)标测、腔内单极电图 (UECG)和体表12导联同步心电图 (ECG)3种方法研究心室复极离散度。结果UECG测值 (UQ_Td,33±7ms)大于MAP测值 (RTd,27±6ms ,P<0.01) ,而小于体表心电图测值 (Q_Td,38±7ms ,P<0.01) ,即Q_Td>UQ_Td>R_Td,但UQ_Td 与R_Td、UQ_Td 与Q_Td、R_Td 与Q_Td 均呈显著线性相关 (r=0.75、0.87 ,0.78 ,P均<0.01)。结论体表心电图Q_Td Objective To evaluate reliability of methods in reflecting ventricular repolarization dispersion.Methods Monophasic action potentials(MAP),unipolar endocardial electrogram(UECG) and simultaneous surface 12 _ lead electrocardiogram(ECG) were recorded in 19 patients without structural heart disease.Results Q _ T dispersion measured from UECG(UQT d) was larger than repolarization time dispersion obtained from MAP recording(RT d)(33±7ms vs.27±6 ms,P<0.01) and was less thanthat assessed from surface ECG(QT d)(38±7mV,P<0.01),that is Q _ T d>U Q _ T d>RT d,but significant linear correlationsexisted between UQ _ T d and R _ T d,UQ _ T d and Q _ T d,Q _ T d and R _ T d(r=0.75,0.87,0.78,respectively,P<0.01).Conclusion Q _ T d measured from surface ECG could legitimately be used to reflect dipsersion of ventricular repolarization.
