摘要
目的通过分析良性阵发性位置性眩晕患者血脂代谢特点,探讨良性阵发性位置性眩晕的发病机制与血脂代谢异常的关系。方法回顾收集的60例良性阵发性位置性眩晕患者血清中甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL)、高密度脂蛋白胆固醇(HDL)、载脂蛋白A1(Apo-A1)、载脂蛋白B(Apo-B),同时以60例同期年龄和性别匹配的排除良性阵发性位置性眩晕的正常体检者的血脂水平作为对照,进行统计学分析。实验组患者和对照组正常人均进行了一个完整的音频前庭测试实验,包括Dix-Hallpike实验和Roll-test实验,测试后半规管(PSC)和水平半规管(HSC)。结果良性阵发性位置性眩晕患者TG、TC、LDL水平较高于对照组,Apo-A1水平较低于对照组,差异有统计学意义(P<0.05);2组间HDL、APOB水平差异无统计学意义(P>0.05)。Logistic多因素分析显示TG是BPPV发病的主要危险因素,APOA1是本病发病的保护因素。结论良性阵发性位置性眩晕患者血脂水平高于正常群体,TG与APOA1水平与良性阵发性位置性眩晕的发病率有相关性。
Objective To analyze the patients with benign paroxysmal positional vertigo lipid metabolic characteristics, and to explore the relationship between benign paroxysmal positional vertigo and dyslipidemia. Methods Retrospective analysis of serum tri glyceride (TG), total cholesterol (TC), low density tipoprotein cholesterol (LDL), high density lipoprotein cholesterol (HDL), apoli poprotein A1 (APOA1), apolipoprotein B (APOB) content were performed in 60 cases with benign paroxysmal positional vertigo;and the blood lipid level were analyzed in normal 60 cases during the same period as the control. Results TG, TC, LDL level of benign par oxysmal positional vertigo group were higher than those in normal hearing patients, APOA1 level was lower than that in normal hearing patients (P 〈 0.05) ; there is no statistical difference in HDL and APOA1 level between two groups (P 〉 0.05) ; Logistic multiple factors analysis showed that TG was a major risk factor for the incidence of'benign paroxysmal positional vertigo, while APOA1 was the protec tive factor. Conclusion The lipid levels of patients with benign paroxysmal positional vertigo are higher than normal control groups, the level with TG APOA1 has a correlation with the incidence of benign paroxysmal positional vertigo.
