腹部创伤患者血清瘦素水平监测及相关性分析

Significance of detecting serum leptin for injury assessment of abdominal trauma

目的检测不同程度腹部创伤患者血清瘦素水平及其在治疗过程中的动态变化并探讨其意义。方法将本科室收治的60例腹部创伤患者依据AIS-ISS评分分为轻伤、重伤、严重伤三组,并设置正常对照组,分别于入院后即刻及第1、2、4、和7 d(T1、T2、T4、T7)共5个时间点,采用放射免疫法检测患者血清瘦素及C-反应蛋白水平并同步进行SIRS评分,对所得结果进行统计学分析及相关性分析。结果不同程度创伤患者血清瘦素水平均升高,平均水平随着伤情加重依次升高,比较差异有统计学意义(P<0.05),其变化与反应创伤严重程度的SIRS等级评分及C-反应蛋白水平相关(P<0.05)。结论血清瘦素可以作为评估腹部创伤患者伤情严重程度及预后的客观指标,其变化可以反映病情的变化。动态监测腹部创伤患者血清瘦素水平有助于准确及时地评估病情和判断预后。

Objective To probe into the relationship between variation of Leptin and severity of illness in abdominal trauma Patients.Methods Sixty patients were divided into mild group and severe group according to ISS score and the severe group was separated by those with complications.Concentration of Leptin and CRP were detected when they were admitted to emergency room as well as 1nd,2th,4th and 7th day in the morning.Meanwhile,body temperature,heart rate,respiratory rate and leukocyte count,were recorded and SIRS scoring was made according to them.Results Leptin increased obviously after abdominal trauma and its increasing extent had correlation to trauma score, Variation of Leptin had correlation to ranking score of SIRS and CRP.Conclusion Concentration of Leptin could be an objective index for evaluation of severity and prognosis in abdominal traumatic patients.Variation of Leptin could reflect the change of illness in abdominal traumatic patients.
