术后小肠瘘的外科综合治疗

Surgical Treatment of Enteric fistula after operation

目的探讨腹部手术后肠瘘的有效治疗措施。方法对1998-01—2013-05收治的26例术后小肠瘘的临床表现、治疗情况进行总结,并结合文献分析。结果 18例保守治疗,治愈率94%(17/18),8例手术治疗,治愈率87%(7/8),全组死亡率8.3%(2/26)。结论早期诊断,采取以及时恰当手术,彻底引流,控制感染,纠正内稳态失衡,应用生长抑素及加强营养支持治疗,绝大部分肠瘘和吻合口瘘均能自愈。

Objective To evaluate the effective treatment and measures of Enteric fistula after operation.Methods The date of clinical manifestations and management of external small intestinal fistula of 26 cases admitted to department of general surgery from January 1998 to May 2013 were analysed and the literatures were reviewed.Results 8 cases underwent re-operation and 1 case died;18 cases did not operate and 1 case died .Conclusion Most patient could be cured by comprehensivw measures including ear;y diagnosis,timely thorough drainage proper operation,control of intection,redressing the balance of internal circumstances,suitable support of nutrition,using Somatostatin and other comprehensive measures .
