摘要
目的探讨智能化(SmartCare/PASB)撤机模式在重症患儿中的应用价值。方法将40例上机的重症手足口病患儿,随机均分为SmartCare/PASB组(SC组)和同步间歇指令通气(SIMV)+压力辅助自主呼吸(PASB)组(SP组),待病情稳定满足临床撤机条件后,分别采用SmartCare/PASB和SIMV+PASB模式撤机。记录撤机时间、总通气时间(TVT)、人工操作次数及再插管率、气管切开率、气胸发生率和呼吸机相关肺炎(VAP)的发生率。结果与SP组比较,SC组的撤机时间、TVT明显缩短、人工操作次数明显减少(P<0.05)。两组患儿再插管成功率、气管切开率、气胸发生率和VAP发生率差异均无统计学意义。结论 SC撤机模式同样适用于上机的重症患儿,可缩短上机患儿撤机时间、TVT和减轻医务人员工作量。
Objective To explore the application of intelligent (Smart Care/PASB ) weaning mode in critical care children. Methods Fourty children with severe hand-foot-mouth disease were randomly divided into Smart Care/PAsB mode group (group SC) and synchronized intermittent mandatory ventilation (SIMV) plus pressure assisted breathing (PASB) group mode (group SP). The weaning time, total duration of ventilation (TVT), manual operation times were recorded. The incidence of reintubation, tracheotomy, pneumothorax and ventilator associated pneumonia(VAP) were also recorded. Results Compared with group SP, weaning time and TVT in group SC was significantly less, manual operation times as significantly reduced (P〈0.05). The rate of success intubation, the incidence of tracheotomy, pneumothorax and VAP in two groups were not statistically significant.Concision SC weaning mode can be applied in critical care children, which can reduce the weaning time, TVT and the workload of medical staff.
