足Lisfranc关节隐匿轻微骨折的CT诊断价值

The diagnostic value of CT in occult and subtle fractures of lisfranc joint

目的探讨多层螺旋CT对足Lisfranc关节隐匿轻微骨折的诊断价值。方法收集首诊时常规X线平片漏诊、而多层CT确诊的足Lisfranc关节隐匿轻微骨折36例,分析其主要CT表现。结果 36例足Lisfranc关节隐匿骨折的主要CT表现是单发为主的轻微裂纹骨折、微小撕脱骨折及少部分轻度凹陷骨折,且多累及中间柱。结论足Lisfranc关节隐匿轻微骨折常规X线平片易漏诊,而多层CT可较准确地显示其损伤细节,具有诊断和指导治疗的较高实用价值。

Objective To explore the diagnostic value of multi-slice CT（MSCT） in the occult and subtle fractures of lisfranc joint .Methods The clinical data was collected in 36 patients with occult and subtle fractures of lisfranc joint which were missed on conventional X-ray film at first diagnosis but diagnosed by MSCT and the main CT manifestations were analyzed .Results In 36 cases of occult and subtle fractures of lisfranc joint the main MSCT manifestations included single subtle crack fracture and small avulsion fractures and a small part of mild depressed fracture,and more involvement of the middle column .Conclusion On conventional X-ray plain film the diagnosis of the occult and subtle fractures of lisfranc joint was easily missed ,but MSCT can accurately show the details of its dam-age,and has the great practical value in the diagnosis and the guide of treatment .
