Expression of claudin-4 and KLF-5 in gastric carcinoma and their clinical implication

目的探讨claudin-4与KLF-5在胃癌组织中的表达及其对胃癌患者预后的临床意义。方法采用免疫组化方法检测142例胃癌组织标本中的claudin-4和KLF-5表达,卡方检验分析其在不同临床病理参数间的差异性,生存分析其对患者预后的影响。结果①肿瘤>2 cm、Ⅲ-IV、T3-T4的患者claudin-4表达阳性率更高;肿瘤>2 cm、有淋巴结转移、T3-T4的患者KLF-5表达阳性率更高(P<0.05);②claudin-4与KLF-5蛋白表达呈正相关关系,有统计学意义(χ2=62.259,P=0.000,r=0.662);③claudin-4、KLF-5阴性表达组平均生存时间明显长于阳性表达组(P<0.05);浸润深度、淋巴结转移、临床分期、claudin-4及KLF-5表达是胃癌5年生存率的独立危险因素。结论 claudin-4与KLF-5的阳性表达与更严重的胃癌病理过程有关;可能直接或者通过调控信号转导刺激了胃癌细胞的浸润。

Objective To investigate claudin-4 and KLF-5 expression in gastric cancer and their clinical significance for the prognosis of patients with gastric cancer.Methods Immunohistochemical methods was employed to detect the expression of claudin-4 and KLF-5 in 142 gastric carcinoma specimens.The chi-square test was used to analyze the differences between clinicopathological parameters,survival analysis was used to explore its effect on the prognosis of patients.Results （1）In the tumor which were ＞ 2 cm,Ⅲ-Ⅳ,T3-T4,claudin-4 expression positive rate was significantly higher; In the tumor which were ＞ 2 cm,T3-T4 and had lymph node metastasis,KLF-5 positive rate was significantly higher （P ＜ 0.05） ; （2） Claudin-4 and KLF-5 expression was positively correlated,with statistically significant （x2 =62.259,P =0.000,r =0.662） ; （3） The disease free survival of claudin-4 and KLF-5 negative expression group was significantly longer than that of the positive group （P ＜ 0.05） ; The depth of invasion,lymph node metastasis,clinical stage,claudin-4 and KLF-5 expression were the independent risk factors for the 5-year survival of gastric cancer patients.Conclusions Claudin-4 and KLF-5 expression is associated with more severe pathological process of gastric cancer,which may be either directly or through the regulation of signal transduction to stimulate invasion of the gastric cancer cells.
