彩色多普勒血流显像、经颅多普勒和数字减影血管造影对锁骨下动脉盗血综合征的诊断价值

Diagnostic value of color doppler flow imaging,transcranial doppler and digital subtraction angiography on subclavian steal syndrome

目的探讨彩色多普勒血流显像(CDFI)、TCD和DSA对锁骨下动脉盗血综合征(SSS)的诊断价值。方法对临床诊断的35例SSS患者分别进行CDFI、TCD和DSA检查。结果 CDFI检查发现,锁骨下动脉中度狭窄15例,重度狭窄12例,闭塞8例;动脉粥样硬化斑块30例,大动脉炎2例。TCD检查显示,Ⅰ期盗血12例,Ⅱ期盗血16例,Ⅲ期盗血7例。DSA检查发现,轻度狭窄6例,中度狭窄13例,重度狭窄10例,闭塞6例。结论 CDFI、TCD和DSA检查对于SSS的诊断各有特点,可互为补充,但最后确诊及指导手术治疗DSA是必不可少的。

Objective To investigate the diagnostic value of color doppler flow imaging （CDFI）,transcranial doppler （TCD） and digital subtraction angiography （DSA） on subclavian steal syndrome （SSS）.Methods The CDFI,TCD and DSA were performed in 35 patients diagnosed as SSS.Results CDFI detection showed that subclavian artery was moderate narrowed in 15 patients,severe narrowed in 12 cases and occluded in 8 cases; and there were atherosclerotic plaque in 30 cases,inflammation of the artery in 2 cases.TCD examination showed that Ⅰ period steal was in 12 cases,Ⅱ period steal was in 16,Ⅲ period steal was in 7 cases.DSA detection showed 6 cases of mild narrow,13 cases of moderate narrow,10 cases of severe narrow and 6 cases of occlucion.Conclusions To the CDFI,TCD and DSA on diagnosing SSS,each has its own characteristics,but can complement each other.DSA is necessary for the final diagnosis and guide surgical treatment.
