角膜地形图引导下不同散光轴向切口白内障手术对角膜散光的影响

The impact of different astigmatism axial incisions on refractory error after cataract surgery

目的研究不同散光轴向切口白内障术对角膜散光的影响。方法 60例白内障患者,随机分成A、B两组各30例(30只眼)。在Orbscan检测出的角膜散光引导下,A组行3.2 mm角膜散光轴向切口,B组行双轴向切口。由同一位手术医师行超声乳化白内障吸除人工晶状体植入术。观察两组患者手术前后角膜散光的变化。结果各组手术前后不同时间点Polar K比较,A组术前与术后2周、1个月、3个月Polar K比较,差异有显著统计学意义,术后3个月比术前减少角膜Polar K 0.23左右。B组术前与术后2周、1个月、3个月的Polar K比较,差异有显著统计学意义,术后3个月比术前减少角膜Polar K 0.34左右,且两组之间Polar K的变化在各个观察时间段的差异无统计学意义。结论对于术前角膜Polar K为0.9左右的患眼,3.2 mm单散光轴向切口与双轴向切口白内障手术都可以矫正部分角膜散光,但差别不大。

Objective To study the impact of different astigmatism axial incisions on refractory error after cataract surgery. Methods We collected 60 cases of cataract patients and randomly divided them into group A and group B. Each group had 30 eyes. Group A patients underwent 3.2 ram corneal astlgnmfism axial incisions guided by Orbscan. Group B patients received double axial incisions. The same physician conducted all surgeries. We observe the postoperative changes of corneal astigmatism between two groups. Results The Polar K results on 2 weeks and 3 months were significantly differ- ent to those before surgery in group A. The 3.2ram corneal astigmatism axial incision reduced about 0.23 of Polar K. In group B, the Polar K results on 2 weeks, 1 month and 3 months also exhibited significant difference to those before the sur- gery. Two astigmatism axial incisions reduced about 0.34 of Polar K. There was no significant difference in Polar K Results between two groups at any time point. Conclusion Both 3.2mm corneal astigmatism axial incision and double axial inci- sions can partly correct corneal astigmatism on patient whose corneal Polar K is about 0.9. But there is little difference be- tween the two incision methods.
