不同性别2型糖尿病患者甲状腺功能与血脂关系分析

Analysis of relationship between thyroid function and lipid level in patients with type 2 diabetes mellitus by gender

目的探讨2型糖尿病患者不同性别及甲状腺功能状态下血脂的特点。方法收集279例2型糖尿病患者，依据性别、促甲状腺激素（TSH）水平分组后，比较各组血脂的差别，应用相关分析研究TSH与血脂的关系。结果（1）女性TSH、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）和载脂蛋白A1（apoA1）高于男性（P＜0．05）。（2）亚临床甲状腺功能减退患者的血脂高于甲状腺功能正常者（P＜0．05）。（3）甲状腺功能正常的2型糖尿病患者，与TSH＜2．5mU/L组相比，TSH为2．5～4．0mU/L组男性HDL-C降低，女性TC升高（P＜0．01）。（4）2型糖尿病患者无论男女TSH均与TC、三酰甘油（TG）呈正相关（P＜0．05）。结论2型糖尿病患者甲状腺功能与血脂水平密切相关，对血脂异常应注意甲状腺功能监测。

Objective To explore the characteristics of lipid level at different thyroid function status in pa-tients with type 2 diabetes mellitus （T2MD） by gender.Methods 279 patients with T2DM were enrolled and divid-ed into groups according to the thyroid stimulating hormone （TSH） and sex.The lipid level was measured and the cor-relation between TSH and lipid profile was analyzed .Results The serum TSH,total cholesterol（TC）,high-density lipoprotein cholesterol （ HDL-C） ,low-density lipoprotein cholesterol （ LDL-C） and apoA1 in female group were signifi-cantly higher than those of male group （P〈0.05）.Among euthyroid patients with T2MD,the male group with TSH 2. 5~4.0 mU/L had a lower HDL-C while female group had a higher TC （P〈0.01） compared to TSH 〈2.5 mU/L group.TSH was positively correlated with serum TC and triglyceride （ TG ） , both in male and female patients withT2DM.Conclution The thyroid function is closely associated with lipid level in patients with T 2DM.TSH Moni-toringis is beneficial to lipid control and prevention of its complications .
