重复经颅磁刺激结合艾司西酞普兰治疗抑郁症临床研究

Repetitive Transcranial Magnetic Stimulation Combined with Escitalopram Treatment of Depression

目的:探讨重复经颅磁刺激(rTMS)结合艾司西酞普兰治疗抑郁症的临床疗效。方法:将68例首发抑郁症患者随机分为对照组和观察组各34例,观察组采用重复经颅磁刺激结合艾司西酞普兰治疗,对照组采用艾司西酞普兰治疗。治疗时间为8周,于8周末评价其疗效,评价指标采用24项汉密顿抑郁量表(HAMD-24)评定。观察各量表评分相对基线值的变化情况。结果:治疗前rTMS结合艾司西酞普兰组HAMD评分[(32.67±6.52)分]与艾司西酞普兰组[(30.88±6.60)分]的差异无统计学意义(t=1.127,P=0.264);治疗第8周末,rTMS结合艾司西酞普兰组23例治愈,8例好转,3例无效;艾司西酞普兰组16例治愈,9例好转,9例无效。两组疗效差异有统计学意义(F=7.843,P=0.005)。治疗后rTMS结合艾司西酞普兰组HAMD评分[(12.20±8.97)分]与艾司西酞普兰组[(19.35±11.43)分]的差异有统计学意义(t=-2.87,P=0.006)。结论:重复经颅磁刺激结合艾司西酞普兰治疗抑郁症患者效果明显,值得临床推广应用。

Objective：To discuss repetitive transcranial magnetic stimulation （rTMS） combined with escitalopram treatment the clinical curative effect of patients depression. Methods： Randomly 68 patients with first-episode depression divided into control group and observation group, the observation group use the rTMS combined with escitalopram ,and the control group adopts escitalopram treatment . After treating 8 W, evaluate its curative effect in 8 weekends, evaluation index with 24 counts of Hamilton depression scale （HAMD - 24）. Observe the changes of the rating scale relative to the baseline value. Results：Former rTMS treating combined between escitalopram group of HAMD score [-（32.67±6.52）] and escitalopram group [（30.88±6.60）] had no statistically signifi- cant （t= 1. 127, P=1. 127） ; treated 8 weekends, rTMS combined with escitalopram citalopram group 23 cases were cured, 8 cases were improved, 3 cases ineffective; escitalopram group of 16 cases were cured, 9 cases were improved, 9 cases of invalid. The differ- ence of the two groups had statistically significant （F= 7. 843, P = 0. 005） . After treated the rTMS combined with escitalopram group of HAMD score [-（12. 202±8.97）] and escitalopram group E（19. 35±11. 43）] there difference had statistically significant （t= 2.87,P=0. 006） . Conclusion： The repetitive transcranial magnetic stimulation combined with escitalopram treatment of patients with depression effect is obvious, and worthy of promotion.
