126例输卵管积水性不孕术后疗效的临床分析

Clinical analysis on the effects of treatments after the surgery for 126 patients with hydrosalpinx infertility

目的探讨输卵管积水造口术后综合治疗对不孕症患者妊娠结局的影响。方法对本院126例双侧输卵管积水性不孕症患者进行回顾性分析。患者均行腹腔镜下双侧输卵管造口术,根据术后采取的措施不同分为两组:A组67例术后仅行输卵管通液;B组59例术后行综合治疗(输卵管通液、理疗及中药灌肠),后者根据输卵管积水直径分为C组(输卵管积水直径<3cm,28例)和D组(输卵管积水直径≥3cm,31例),分别记录术后1年的临床妊娠情况。结果 B组临床妊娠率高于A组,C组高于D组,差异均具有统计学意义(P均<0.05)。结论输卵管积水造口术后综合治疗,可有效提高不孕症患者妊娠率,但输卵管积水直径对不孕症患者术后妊娠率有一定的影响。

Objective To discuss the effects of comprehensive treatment following salpingostomy on the pregnancy out- comes of patients with hydrosalpinx infertility. Methods A total of 126 patients with bilateral hydrosalpinx infertility who were admitted to our hospital were analyzed retrospectively. The patients were all received bilateral salpingostomy laparoscopically and were assigned to two groups according to different postoperative measures. Group A （76 patients） was only received hydrotubation, and group B （59 patients） was received a series of comprehensive treatment （hydro- tubation, physiotherapy and enema by TCM）. Group B was also divided into group C （diameter of hydrosalpinx 〈3 cm, 28 patients） and group D （diameter of hydrosalpinx I〉3 cm, 31 patients） according to the diameter of hydrosalpinx. Conditions of clinical pregnancy of the four groups were recorded within one year after the surgery. Results The clini- cal pregnancy rate in group B was higher than that in group A, and group C higher than group D. The differences were all statistically significant （P〈0.05）. Conclusion Comprehensive treatment following salpingostomy in treating hydros- alpinx helps effectively improve the pregnancy rate of patients with infertility, whereas the diameter of hydrosalpinx shows certain effects on the postoperative pregnancy rate of patients with infertility.
