CHiAO系列胆道镜在取息肉保胆手术中对胆囊息肉性质的判断与处理

THE JUDGMENT OF THE NATURE OF THE GALLBLADDER POLYPS FROM GALLBLADDER-PRESERVING POLYPECTOMY WITH COMPACT CHOLECYSTOSCOPE AND ITS MANAGEMENT

目的探讨CHiAO系列胆道镜在取息肉保胆手术中对胆囊息肉性质的判断方法及处理方式。方法2008年1月-2012年3月间，对231例胆囊息肉患者行CHiAO系列硬质胆道镜取息肉保胆手术，采用术中CHiAO硬质胆道镜下观察、CHiAO硬质超声胆囊内镜检查、术中染色胆囊镜检查综合判断息肉的良恶性，根据判断结果采取不同的处理方式，并与术后病理检查结果对比。结果231例胆囊息肉术中综合判断均为良性，未发现可疑胆囊癌病例，全部成功实施了取息肉保胆手术。术后病理检查结果：183例为胆囊胆固醇性息肉，其中15例伴有不典型增生；2例炎性息肉，46例胆囊腺瘤，无胆囊癌病例。结论采用CHiAO系列硬质胆遁镜在取息肉术中能对胆囊息肉的良恶性作出准确判断，并可根据判断结果进行有效处理。

Objective To investigate how to judge the nature of the polyps from gallbladder - preserving polypectomy with compact cholecystoscope and its management. Methods During Jan 2008 to Mar 2012, gallbladder polyps patients were checked by ultrasound pre the operation, and compact cholecystoscope, EUS as well as staining cholecystoscopy during the operation, then pathological examination post the operation, under all the above examina- tion, the nature of the polyps were judged and the treatment were decided. Results 231 cases of operation were all executed successfully. If the nature of the polyps were benign or malignant could be judged initially by the ultrasound cheek pre the operation and the compact cholecystoscope, EUS as well as staining cholecystoseopy during the opera- tion, the resuhs was confirmed by the pathological examination post the operation, and the consistent rate was 100%. Conclusion The nature of the polyps were benign or malignant could be judged initially by the ultrasound check pre the operation and the compact choleeystoscope, EUS as well as staining eholecystoscopy during the operation, the results was confirmed by the pathological examination post the operation, accordingly, modus operandi could be chose.
