摘要
目的:探讨实时超声组织弹性成像在颈部淋巴结疾病诊断中的价值。方法:分析101例颈部淋巴结肿大患者的155个病灶行实时组织弹性成像。获取弹性成像图后,进行弹性成像评分、测量弹性成像应变率比值、弹性面积与二维面积比值。所有目标淋巴结均经术后病理或细针抽吸细胞学检查证实。结果:颈部恶性转移性淋巴结评分较高,良性淋巴结及原发恶性淋巴结评分较低,差异具有统计学意义(P<0.05)。绘制受试者工作曲线(ROC)计算出SR比值的最佳临界值,以1.67为界值来鉴别颈部良恶性淋巴结,其灵敏度、特异度分别为82.27%、82.23%。颈部良性淋巴结弹性面积与二维面积之比(1.04±0.43),颈部转移性淋巴结弹性面积与二维面积之比约(1.15±0.92),差异具有统计学意义(P<0.05)。结论:超声弹性成像可以对颈部淋巴结疾病诊断提供有价值的信息。
Objective To discuss value of real- time ealstography in the diagnosis of cervical lymph node disease. Method RTE were performed for 153 lymph nodes in 101 patients. Elastographic grade,strain ratio( SR), the ratio of the elastic area and two dimensional area were acquired in elastograms. All lymph nodes were confirmed by pathology or fine - needle aspiration cytology. Results Metastatic lymph nodes with elastographic grades were higher in malignant lymph nodes. Reactive lymph nodes with elastographic grades were lower in benign lymph nodes. The difference was statistically significant ( P 〈 0. 05 ). SR: 1.67 as threshold to identify benign and malignancy lymph nodes of neck by Receiver Operating Characteristic (ROC) ,its degree of sensitivity, specific were 82.27% ,72. 23%, respectivedly. The ratio cer- vical benign lymph nodes elastic area and of the two dimensional area was about ( 1.04 ± 0. 43), The ratio of the elastic area and two di- mensional area of cervical malignant lymph nodes was about ( 1.15 ± 0. 92 ), The difference was statistically significant ( P 〈 0. 05 ) . Conclusion Ultrasound elastography can provide valuable information in diagnosing cervical lymph nodes.
