老年高血压伴肥胖患者血清脂联素水平的变化及其影响因素

The changes and affecting factors of serum adiponectin in obese eldly patients with hypertension

目的探讨老年高血压伴肥胖患者血清脂联素水平的变化及其影响因素。方法 118例患者分为高血压肥胖组、单纯高血压组、单纯肥胖组及正常对照组。测量其一系列生化指标及血清脂联素水平,进行逐步回归分析,找出影响因素。结果四组的年龄、性别比例比较,差异无统计学意义。前三组患者的体质指数、总胆固醇、低密度脂蛋白胆固醇与正常对照组比较明显升高;前三组患者的高密度脂蛋白胆固醇与正常对照组比较明显降低;高血压肥胖组、单纯高血压组患者的空腹血糖、收缩压、舒张压与正常对照组比较明显增高;高血压肥胖组和单纯高血压组患者的体质指数、收缩压、舒张压比单纯肥胖组明显升高;高血压肥胖组患者的体质指数、空腹血糖、总胆固醇、甘油三酯比单纯高血压组明显升高。高血压肥胖组、单纯高血压组、单纯肥胖组、对照组患者的血清脂联素水平逐渐升高。BMI、SBP是影响患者血清脂联素水平的显著因素。结论老年患者的高血压、肥胖状态可能会导致体内血清脂联素水平降低,进而引起低脂联素血症。

Objective To investigate the change and its influence factors of serum adiponectin levels in obese patients with hypertension. Methods A total of 118 patients were divided into obese hypertension group, hypertension group, simple obesity group and normal control group. Measuring a series of biochemical indexes and serum adiponectin level, then the stepwise regression analysis, find out the influencing factors. Results Age, sex ratio of four groups had no sig- nificant differences. BMI, TG, LDL-C of four groups compared with the control group increased significantly. HDL-C of four groups compared with the control group decreased significantly.FPG, SBP, DBP of obese hypertension group, hypertension group compared with the control group increased significantly. BMI, SBP, DBP of obese hypertension group and hypertension group were significantly were higher than in simple obesity, hypertensive obese group. BMI, FPG, TG, TC were significantly higher than the simple hypertension group. Serum adiponectin levels in obese hyperten- sion group, hypertension group, simple obesity group, the patients in the control group increased gradually. BMI, SBP were the most important influencing factor. Conclusion Hypertension, obesity will reduce serum adiponectin induced, and cause hypoadiponectinemia.
