低场磁共振SSFSE MRCP序列对鉴别肝血管瘤和肝囊肿的应用价值 被引量:2

摘要 目的探讨低场磁共振胰胆管水成像(MRCP)序列对鉴别肝血管瘤和肝囊肿的应用价值。方法对已进行MRI平扫加增强扫描明确诊断的患者进行MRCP成像检查,包括肝海绵状血管瘤75例、肝囊肿66例、肝海绵状血管瘤合并肝囊肿26例,并与MRI增强扫描结果进行对照。结果 MRCP成像诊断肝海绵状血管瘤病灶169个,诊断符合率95.48%;诊断肝囊肿病灶156个,诊断符合率97.50%;总体鉴别正确325个病灶,鉴别正确率96.44%。全部病例T2WI呈高信号或较高信号,T1WI呈低信号,STIRT2压脂呈明显高信号。MRCP序列肝血管瘤信号明显衰减甚至消失,而囊肿病灶呈明显高信号,二者形成非常显著的差异。结论磁共振水成像充分利用了水的静态特征,免除了增强检查鉴别的繁琐,简便易行,且费用低,具有临床推广价值。 Objective To investigate the value of MRCP sequence in low field MRI in differentiating between cav- ernous hemangioma of liver(CHL) and hepatic cyst(HC). Methods Seventy-five CHL patients, 66 HC patients and 26 CHL patients accompanied with HC were definite diagnosed with plain and enrolled in this study. Subsequently,all pa- tients underwent magnetic resonance cholangiopancreatography(MRCP). The results on MRCP were compared to diag- nosis with enhanced MRI scanning. Results CHL and HC lesions both exhibited high signal intensity or slightly high signal intensity on T2-weighted imaging and low signal intensity on Tl-weighted imaging. On STIRT2 imaging, they also both exhibited obvious high signal intensity. However, CHL and HC lesions remarkably showed different intensity on MRCP imaging. CHL lesions showed attenuated or no intensity while HC lesions showed obvious high intensity. 169 HC lesions and 156 CHL lesions were identified by MRCP imaging. The coincidence rates were 95.48% and 97.50% respectively. In total, 325 lesions identified by MRCP imaging exactly and the coincidence rate was 96.44%. Conclu- sion MRCP sequence in low field MRI is cheap,convenient and reliable technique to differentiate between CHL and HC with using the static characteristics of water.
