摘要
目的探讨桂西铝工业基地居民血清铝(Al)、锌(Zn)、铬(Cr)、镁(Mg)含量与2型糖尿病(T2DM)慢性并发症的关系,分析其在T2DM慢性并发症发生、发展过程中的作用。方法检测居住在桂西铝工业基地的初诊T2DM患者(A组)38例与T2DM并慢性并发症(B组)67例和健康成人(C组)87例的血清Al、Zn、Cr、Mg的含量,通过t检验比较两组间平均水平差异。并通过非条件多因素logistic回归模型分析Al、Zn、Cr、Mg在T2DM慢性并发症致病过程中的作用。结果①与C组比较,B组Al含量均偏高,Zn、Cr、Mg含量偏低,差异均有统计学意义(P<0.05或<0.01);A组和B组Zn含量比较,差异有统计学意义(P<0.01)。②与A组比较,B组的2个亚组的Zn及2型糖尿病并周围神经病变(DPN)组的Cr含量均偏低,经比较差异有统计学意义(P<0.01);Cr在B组的2个亚组间比较,差异有统计学意义(P<0.01)。③将Al、Zn、Cr、Mg纳入非条件多因素logistic回归模型进行分析,结果提示Zn、Cr可能为保护性因素。结论桂西铝工业基地居民血清Zn、Cr水平下降可能与糖尿病并慢性周围神经病变的发生、发展有关。因此,适当补充Zn、Cr可能对防治T2DM慢性并发症有一定的效果。
Objective To study the relationship between the content of blood microelement (AI,Zn,Cr,and Mg)and the chronic complications caused by T2 diabetes mellitus (T2DM) in residents of west Guangxi aluminum industrial base,so as to analyze their effect on the occurrence and development of T2DM chronic complications.Methods The serum content of A1,Zn,Cr,and Mg in 38 patients with newly diagnosed diabetes (Group A),67 cases with T2DM chronic complications (Group B),and 87 healthy adults (Group C) from aluminum industrial base were tested.Their average levels between groups were compared by t-test. Moreover, the mulitiple-factored and non-conditional logistic regression was used to analyze the influence of A1,Zn,Cr,and Mg in the pathogenic process of T2DM chronic compli cations.Results ①Compared with Group C, Group B had high A1 concentration, but had relatively low Zn, Cr and Mg concentration.Such difference was statistically significant ( P 〈0.05 or P〈0.01). There was significant difference in the comparison of Zn concentration between Group A and Group B( P 〈0.01).②Compared with those of Group A, the Zn concentration and Cr concentration of diabetic peripheral neuropathy (DPN group) in two subgroups of Group B were relatively low, so the differences were statistically significant( P 〈0.01),The difference of Zn concentration between two subgroups of Group B was statistically significant ( P 〈0.01).③The analysis of mulitiple-factored and non-conditional logistic regression for AI,Zn,Cr,and Mg showed that Zn and Cr may be the protective factors to chronic complications.Conclusion The decrease of Zn and Cr concentration may be related to the occurrence and development of diabetes and peripheral neuropathy.Thus,to properly supplement Zn and Cr may have certain positive effect on preventing T2DM chronic complications.
