幼稚粒细胞复检标准的建立与评估

Establishment and validation of immature granulocyte review criteria

摘要 目的通过对临床标本常规检测数据进行分析,确定清华大学第一附属医院检验科对全自动血细胞分析仪幼稚粒细胞检测结果的复检筛选标准。方法选择清华大学第一附属医院院门、急诊及住院患者全血标本3 300份,用Sysmex XE-5000全自动血细胞分析仪检测幼稚粒细胞,同时对采用Sysmex SP1000i全自动推片机制备的2张血涂片进行显微镜复检。计算不同含量范围数据的假阴性、假阳性、真阴性、真阳性,以最大可接受假阴性率不能超出5%为选择清华大学第一附属医院实验室幼稚粒细胞的复检筛选标准。选择281例标本验证新确定复检标准的有效性。结果通过对仪器检测结果和显微镜镜检结果的统计学分析,将幼稚粒细胞百分数值1.1为筛选标准,验证试验结果显示,血液病细胞无漏检情况。结论建立适合该实验室的幼稚粒细胞复检的筛选标准,在保证检验结果质量的同时,提高了工作效率。 [ Objective ] To find out the suitable review criteria for medicine laboratory immature granulocyte via the analysis of ex perimental data from Sysmex XE-5000 hematology analyzer in First Affiliated Hospital of Tsinghua University. [ Methods ] A total of 3 300 whole blood samples were collected from First Affiliated Hospital of Tsinghua University, these samples were detected by Sys mex XE-5000 hematology analyzer, and 2 blood smears made by SP1000i were reviewed to observe immature granuloeytc via micro-scope. The false negative ratio ( FN ) , the false positive ratio ( FP ) , the true negative ratio ( TN ) and the true positive ratio ( TP )in each group were calculated. And the maximum FN not over 5% was taken as the laboratory immature granulocyte review criteria in First Affiliated Hospital of Tsinghua University. A total of 281 samples were selected to validate the new review criteria. [ Results] Through the statistical analysis of instrument test results and the results of microscopic examination, immature granulo- cytes 1.1% was the new screening criteria, the validation result demonstrated that no blast cell and immature cell was missed. [ Conclusion] Establishment of new screening criteria for immature granulocyte, which is suitable for laboratory, can not only as sure quality of results but also enhance work efficiency.
