THE VALUE OF ^(18)F-FDG PET/CT IN THE DIAGNOSIS OF PATIENTS WITH INTRACRANIAL SPACEOCCUPYING LESIONS

摘要 目的探讨18F-FDG PET/CT显像在颅内占位性病变诊断中的价值。方法回顾性分析48例颅内占位性病变病人的颅脑及体部18 F-FDG PET/CT显像及临床随访结果。结果 PET/CT诊断脑转移瘤29例,经手术、内镜或穿刺病理学证实原发灶者24例,病理确诊脑转移瘤未明确原发灶者4例,病理证实为结核1例;PET/CT诊断颅内原发恶性肿瘤19例,病理证实为淋巴瘤8例,胶质瘤5例,2例随访中发现原发灶证实为转移瘤,余4例临床随访1年均未见颅外恶性病灶。结论颅内占位性病变病人行18 F-FDG PET/CT显像可有效发现原发病灶,并根据体部及颅脑显像特点初步判定转移性肿瘤和原发性肿瘤。 Objective To evaluate the value of 18F-FDG PET/CT in the diagnosis of intracranial spaceoccupying lesions(ISOL). Methods The cranial and whole-body 18F-FDG PET/CT images and clinical follow-up results in 48patients with ISOL were analyzed retrospectively. Results Twenty-nine patients were diagnosed metastatic brain tumor by 18 F-FDG PET/CT,of which,24were confirmed by endoscopy or puncture biopsies as primary tumor,four were confirmed by histology as metastatic but the primary lesion not yet indentified,one was identified as tuberculosis;nineteen cases were diagnosed as intracranial primary malignant tumors,of which,eight were confirmed by histology as lymphoma,five were glioma,two of them previously diagnosed as primary ones were confirmed as metastatic lesion during follow-up,and the rest four cases were not found any extra-cranial malignant lesions during one-year follow-up. Conclusion Screening patients with ISOL using 18 F-FDG PET/CT can effectively find primary lesions and initially differentiate the nature of the lesions.
