盆炎灌肠方对盆腔炎性疾病的临床疗效及作用机理探讨

The Exploration of Efficacy of TCM Clyster in the Treatment of Pelvic Inflammation and Mechanism of Action

目的：观察盆炎灌肠方保留灌肠治疗盆腔炎性疾病的临床疗效。方法：将入选的120例患者随机分为4组，每组30例，A组予盆炎灌肠方保留灌肠，B组予口服蒲皂愈炎汤，C组予口服蒲皂愈炎汤并保留灌肠，D组予奥硝唑分散片。结果：经14 d治疗后，A组、B组、C组的中医临床痊愈率、显效率和总有效率均明显优于D组（ P＜0.05或P＜0.01）。 A组与C组的显效率分别为50.00％和53.33％（P＞0.05）；A、C组与B组比较（P＜0.05或P＜0.01）。总有效率依次为C组93.33％、A组90.00％、B组76.67％、D组66.67％，A组与C组比较（P＞0.05）；A、C组与B组比较（P＜0.05或P＜0.01）。表明A组和C组的疗效明显优于B组和D组。4组患者治疗后的局部体征评分、疼痛评分、止痛疗效及中医症状评分均较治疗前明显改善（ P＜0.05或P＜0.01）。止痛疗效A组、B组、C组、D组的总有效率依次为86.67％、76.67％、83.33％和73.33％，A组优于其他3组。 A组有2例、C组有1例于1～3 d时出现肛门口轻微不适感，未经特殊处理自行好转；D组有7例有恶心感，加服维生素B6后改善。表明A组无明显不良反应；治疗费4组从少到多依次为D组2.59＆#215;14＝87.63（元）、A组8.41＆#215;14＝114.94（元）、B组10.7＆#215;14＝149.94（元）、C组的费用（8.41＋10.7）＆#215;14＝264.74（元）。结论：盆炎灌肠方保留灌肠治疗盆腔炎性疾病的作用独特、疗效确切、费用低廉。

Objective：To observe the clinical curative effect of TCM Clyster on pelvic inflammation .Methods：the 120 patients were ran-domized into 4 arms：A, B, C and D, 30 cases in each arm.We gave TCM Clyster to Arm A, Puzao Yuyan Decoction to Arm B, TCM Clyster and Puzao Yuyan Decoction to Arm C , Ornidazole dispersible tablet to Arm D .Results：After treatment for 14 consecutive days , the recovery rate , significant effective rate and total effective rate of Arm A , B and C were significantly higher than that of Arm D （ P＆lt;0.05 orP＆lt;0.01）.The significant effective rate of Arm A and C were 50.00%and 53.33%（P＆gt;0.05）, having statistical significance compared with Arm B （P＆lt;0.05 orP＆lt;0.01）.The total effective rate of Arm C, A, B and D were respectively 93.33%, 90.00%、, 76.67%and 66.67%.Arm A was not statistically different than Arm C , however both arms were significant than Arm B （ P＆lt;0.05 orP＆lt;0.01）, which indicated that the efficacy of Arm A and C was superior to Arm B and D .After treatment, all the arms improved in terms of local symptoms, pain score, analgesic efficacy and TCM symptom score（P＆lt;0.05 orP＆lt;0.01）.The total effective rate of anal-gesic efficacy of Arm A.B,C and D were respectively 86.67%、76.67%、83.33% and 73.33%.2 patients in Arm A and 1 patient in Arm C felt slight discomfort in the anus at day 1-3, which recovered spontaneously without corrective treatment .7 patients in Arm D felt nauseated, which was relieved after taking vitamin B 6.Patients in Arm A reported no obvious adverse reactions .Costs of treatment：Arm D：￥87.63, Arm A：￥114.94, Arm B：￥ 149.94, Arm C：264.74.Conclusion：TCM Clyster is effective to treat pelvic inflammation , and less expensive compared to other treatments in this research .
