摘要
目的探讨老年ERCP术后胰腺炎(PEP)患者血清CRP水平变化及其与胰腺炎严重程度的关系。方法选择在我院拟实施ERCP治疗的老年胆管结石患者185例,分为PEP组(21例)和非PEP组(164例)。所有患者均于ER CP术前及术后6、24及48 h检测血清淀粉酶和C反应蛋白水平。结果 PEP组患者术后6、24及48 h血清淀粉酶均明显高于术前水平(P<0.05)。非PEP组患者术后24及48 h血清淀粉酶均明显高于术前水平(P<0.05)。两组间比较,PEP组患者术后各时间点血清淀粉酶水平明显高于非PEP组患者(P<0.05)。PEP组患者术后6、24及48h血清CR P水平均明显高于术前水平。两组间比较,PEP组患者术后6、24及48 h血清CR P水平明显高于非PEP组患者(P<0.05)。术后6、24及48 h轻型胰腺炎患者和重型胰腺炎患者的血清CRP水平逐渐升高,均明显高于术前水平(P<0.05)。两组间比较,术后各时间点重型胰腺炎患者的血清CRP水平明显高于轻型胰腺炎患者(P<0.05)。结论 CRP不仅可作为ERCP术后胰腺炎发病的早期监测指标之一,并且可通过测定CRP浓度高低来预测ER CP术后胰腺炎发病的严重程度。
【Objective】To study the changes of serum CRP level of elderly patients with pancreatitis(PEP)postoperative ERCP and their relationship with the severity of acute pancreatitis. 【Methods】185 elderly patients with biliary calculi in our hospital received ERCP treatment were divided into the PEP group(21 cases) and the noPEP(164 cases), the PEP in the group for severe acute pancreatitis in 8 cases, 13 cases of mild pancreatitis. The serum amylase and c-reactive protein levels of all patients in ERCP preoperative and postoperative 6 h, 24 h, 48 h were detected. 【Results】 The serum amylase levels after 6 hours, 24 hours, 48 hours in PEP group were significantly higher than that of preoperative levels(P 0.05), the serum amylase levels after 24 hours, 48 hours in no-PEP group were significantly higher than that of preoperative levels(P 0.05), the comparison between the two groups, the serum amylase levels PEP patients postoperative 6 hours, 24 hours, 48 hours were significantly higher than the PEP patients(P 0.05). The serum CRP levels after 6 hours, 24 hours, 48 hours in PEP group were significantly higher than that preoperative. The serum CRP level after 6 hours, 24 hours, 48 hours in PEP group was significantly higher than in no-PEP(P 0.05).After 6 hours, 24 hours, 48 hours, the serum CRP level of light and heavy pancreatitis patients increased, were significantly higher than that of preoperative(P 0.05). The serum CRP level of severe acute pancreatitis patients postoperative each time point was significantly higher than that of light pancreatitis patients(P 0.05). 【Conclusion】CRP is not only used as one of the early monitoring indicators of the pathogenesis of pancreatitis postoperative ERCP, and also predict the severity of pancreatitis postoperative ERCP by detecting CRP concentration.
