95例2型糖尿病患者维生素D水平及变化意义 被引量:2

摘要 目的探讨2型糖尿病(type 2diabetes mellitus,T2DM)患者血清维生素D的水平及变化,以及维生素D水平与胰岛素分泌及胰岛素抵抗的关系。方法选取95例住院T2DM患者(T2DM组)及同期87例健康体检者(对照组)为研究对象,测定静脉血浆空腹血糖(fasting plasma glucose,FPG)、餐后2h血糖(2-hour plasma glucose,2hPG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、三酰甘油(triglycercide,TG)、空腹胰岛素(fasting insulin,Fins)、糖化血红蛋白(glycosylated hemoglobin,HbA1c),FPG及Fins值用稳态模式评估法的公式HOMA-IR和HOMA-B估测组织胰岛素抵抗性及胰岛β细胞胰岛素分泌功能并测定血清25-(OH)VD3。比较2组血清25-(OH)VD3的差异,分析血清25-(OH)VD3与HOMA-IR、HOMA-B及各检测指标的相关性。结果 T2DM组FPG、2hPG、HbA1C、TC、TG水平高于对照组(P<0.05),25-(OH)VD3水平明显低于对照组(P<0.05);HOMA-IR与HOMA-B呈正相关(r=0.48,P<0.05),血清25-(OH)VD3水平与HOMA-IR呈负相关(r=-0.08,P<0.05);血清25-(OH)VD3水平与FPG、2hPG、TC、TG、BMI等成负相关(r=-0.724、-0.686、-0.458、-0.412、-0.214,均P<0.05)。结论 2型糖尿病患者存在维生素D缺乏,维生素D水平影响胰岛素分泌及胰岛素抵抗。
