小儿输血传播病毒感染状况研究

Transfusion transmitted virus infection in children

目的 研究输血传播病毒 (TTV)感染对小儿各型肝炎 ,健康儿童的影响及与临床的关系。方法 采用PCR方法检测 36 3例各型 (乙型 ,丙型 ,非甲 戊及庚型 ,婴儿肝炎 )肝炎和 2 4例健康儿童的血清TTVDNA ,对阳性者进行血清TTVDNA半定量分析 ,对各组病例的临床特征进行对比研究。结果 (1) 36 3例各型小儿肝炎TTVDNA检出率为 8 8% (32 / 36 3例 )。其中乙型肝炎占 10 8%(2 5 / 2 32例 ) ,丙型肝炎占 5 % (4 / 74例 ) ,非甲 戊、庚型肝炎占 7% (2 / 2 9例 ) ,婴儿肝炎占 4% (1/ 2 8例 )。 (2 ) 2 4例健康儿童TTVDNA检出率为 4% (1/ 2 4例 )。 (3)TTVDNA阳性者与阴性者在性别、年龄、病程、丙氨酸转氨酶 (ALT)异常、血清胆红素升高方面无差别。前者有输血制品史的多于后者(χ2 =5 2 86 ,P <0 0 0 1)。 (4 )对血清TTVDNA阳性者进行半定量分析 ,发现不同滴度病例的临床特征无差别。结论 TTV感染在各型小儿肝病和健康儿童中广泛存在。其主要传播途径为经血液传播。

Objective To study the clinical features of transfusion transmitted virus (TTV) infection in children with hepatitis B, C, non A G and infantile hepatitis syndrome as well as in healthy children Methods Serum TTV DNA was detected in 363 cases of hepatitis and 24 healthy children using polymerase chain reaction (PCR). Results The serum TTV carriage rate was 8 8% in the 363 cases, and 10 8% in hepatitis B cases, 5% in hepatitis C, 7% in non A G hepatitis, 4% in infantile hepatitis syndrome patients and 4% in healthy children (χ 2=3 271, P >0 05) TTV DNA was (+) in 11 cases (33%), (++) in 9 cases (27%), and (+++) in 13 cases (39%) No significant difference was noted in sex, age, duration of the disease and serum levels of ALT and bilirubin between those whose serum TTV DNA was positive and negative The number of patients with positive history of blood transfusion was greater in seropositive cases than in seronegative cases (χ 2=52 86, P <0 001) Conclusion TTV infection seemed to be common in pediatric cases of hepatitis and healthy children The common route of transmission is blood transfusion Coexistence of TTV infection and a known hepatitis virus infection in children does not seem to be associated with existence or severity of liver damage.
