脊髓内血管母细胞瘤的MRI诊断 被引量:3

摘要 目的评估脊髓内血管母细胞瘤的MRI表现。方法本文回顾分析了 8例经手术及病理证实的髓内血管母细胞瘤。使用场强为 0 35T(特斯拉 )的超导型MR成像仪 (SE系列 ,多回波技术 )对全部病例行矢状面、横断面T1加权成像和矢状面T2加权成像。 5例行Gd -DTPA增强扫描 ,剂量为 0 1mmol/kg·b w ,静脉注射。结果 8例中 ,7例位于颈髓内 ,只有 1例位于胸段。MRI成像中所有病例均显示受侵脊髓弥漫性增粗。与正常脊髓信号比较 ,T1加权成像呈等低信号 ,T2 加权成像呈高信号。 5例行Gd -DTPA增强T1加权成像 ,所有病变呈现一伴随囊腔的界限清楚的强化肿块。全部病例均见迂曲的血管流空现象。结论MR图像可多平面成像 ,适于评估脊髓肿瘤。Gd -DTPA增强MR成像能在大的囊变区精确确认多血管的肿瘤病灶。 Objective To evaluate the MR imaging of intramedullary hemangioblastoma Methods This paper reviewed the MR findings in 8 patients with surgically and pathologically proved intramedullary hemangioblastoma All scans were obtained on a 0 35 Tesla supercondutive MR unit, and spin echo and multiple echoes were applied All the patients were made sagittal and axial T 1-weighted images(T 1WI) Sagittal T 2 -weighted images were also taken The intravenous Gd-DTPA was administered in a dosage of 0 1 mmol/kg·b w in 5 patients Results Seven lesions were in the cervical spinal cord, only one in the thoracic segment Extensive widening of leisioned spinal cords could be shown in all cases on MRI Signal on T 1-weighted images (T1WI) of the tumors demonstrated hypointensity to isointensity in contrast with the normal spinal cord, and tumors had higher intensity signal than that of the normal spinal cord on T2WI Gd-DTPA enhanced T1WI were obtained in 5 cases All lesions presented a well-demarcated, enhanced mass that accompanied syrinx Tortuous vascular signal void could be observed in all cases Conclusion MR imaging is well suited for evaluation of spinal cord tumors because it provides superior soft- tissue characterization and has multiplanar imaging capabilities Gd-DTPA enhanced MRI allows accurate recognition of the highly vascular tumor nidus within a large area of cystic change
