腰间盘突出症的手术与非手术治疗

Operative and Nonoperative Treatment of Prolapse of Lumbar Intervertebral Disc

目的探讨腰间盘突出症的手术与非手术治疗.方法对200例腰间盘突出症的治疗进行回顾.非手术治疗132例,采用卧床骨盆牵引加药物治疗,辅以理疗等.手术治疗68例,采取开窗、半椎板切除,对年龄较大,考虑有椎管狭窄或中央型腰间盘突出较大者采用全椎板切除.结果非手术治疗效果优良者为97例,占74%.手术治疗组全部达到优良标准.结论非手术治疗是腰间盘突出症的最基本治疗方法,可以获得很好的效果.在手术治疗时应严格选择手术适应症,尽量选择对脊柱稳定性影响小的手术方法.

Objective To probe operative and nonoperative treatment of prolapse of lumbar intervertebral disc.Methods 200 patients with prolapse of lumbar intervertebral disc were treated,nonoperative treatment 132 cases were treated with bedrest and traction of pelvis and pharmacotherapy and physiotherapy.Operative treatment 68 cases treated with fenestration or semilaminectomy operation,the old patients with lumbar spinal canal stenosis or central type disc protruding is large treated with laminectomy operation.Results 97 cases(74%)were excellent nonoperative treatment.Operative treatment cases reached excellent standard.Conclu sion Nonoperative treatment is most basic therapy to the prolapse of lumbar intervertebral disc.Operative treatment must select indication strictly,as far as possible select the operative method that is little effect on spinal stabilization.
