儿童副鼻窦的16层CT低剂量扫描 被引量:17

摘要 目的:探讨儿童副鼻窦病变16层螺旋CT检查中的低剂量应用。方法:60例疑诊副鼻窦炎的6~12岁儿童随机分成三组,固定管电压120kV,管电流分别设置为100mAs(A组)、50mAs(B组)和20mAs(C组),获取5mm层厚的轴位图像(标准算法和骨加算法)并0.625mm层厚的冠状位图像。比较三组扫描的容积剂量指数(CTDIv01)和剂量长度乘积(DLP),评价图像质量和图像噪声。结果:三组图像均可满足临床诊断。与A组比较,B、C两组的cTDIvol和DLP分别降低了50%和80%。A、B组间图像质量无显著性差异(P〉0.05),但C组明显低于A、B组(P〈0.01)。三组轴位图像间噪声有显著性差异(P〈001),A、B组冠状位图像间噪声未见显著性差异(P〉0.05),但C组明显高于其他两组(P〈0.01)。结论:应用20mAs管电流进行儿童副鼻窦CT扫描,可以清楚显示副鼻窦结构,同时大幅降低辐射剂量。 Purpose: To investigate the significance of low-dose scan with 16-detector row computed tomography in paranasal sinuses of children. Methods: Sixty children were randomly divided into 3 groups, which were with same tube voltage of 120kV and different tube current of 100mAs(group A), 50mAs (group B) and 20mAs(group C) respectively. Axial images with 5mm thickness and coronal images with 0.625mm thickness were obtained. CTDIvol, DLP, the quality of images, as well as SD of the images from these 3 groups were evaluated. Results: The images of these three groups were all satisfied for the clinical diagnosis. However, the CTDIvol and DLP were all markedly decreased in the group A and group B. Statistical significant difference of the image quality was not found between group A and group B (P〉 0.05), but statistical significant difference was detected between the group A, B and the group C (P〈0.01). The difference of SD values was found with statistical significance among the 3 groups on the axial images (P〈0.01), and between group A, B and group C (P〈0.01) on the coronal images. Conclusion: The low-dose CT scanning of paranasal sinuses in children could be achieved by the method of decreasing tube current to 20mAs, through which the diagnostic demand could be satisfied.
