早中孕期NT增厚和颈部水囊瘤与胎儿心脏畸形的相关性研究 被引量:28

摘要 目的 :探讨早中孕期胎儿NT增厚及颈部水囊瘤与心脏畸形的相关性。方法 :回顾性分析我科室2005年3月—2013年7月早中孕筛查326例NT增厚以及282例出现颈部水囊瘤的胎儿的病例,追踪胎儿心脏结构的情况,分析NT增厚以及颈部水囊瘤胎儿与心脏畸形相关性以及心脏异常的类型分布。结果:以NT≥2.5 mm为界,NT增厚的胎儿心脏畸形的发生率约为4.9%,颈部水囊瘤胎儿伴心脏畸形发生率约14.2%,两者统计学差异明显。NT增厚胎儿心脏畸形发生率与NT厚度正相关,而颈部水囊瘤心脏畸形发生率与囊性包块大小无关。并且两组常见的伴发心脏畸形均为左心系统畸形。结论:NT增厚和颈部水囊瘤与胎儿心脏畸形有明显的相关性,且出现颈部水囊瘤时心脏畸形的发生率更高,早中孕期胎儿NT增厚和出现颈部水囊瘤时要进行针对性心脏超声检查并注重对左心系统畸形的排查。
