脊髓栓系综合征MRI表现

MRI manifestation of tethered cord syndrome

目的探讨脊髓栓系综合征的MRI影像表现特征。方法回顾性分析14例脊髓栓系综合征患者的MRI影像资料。结果 14例MRI均明确诊断为脊髓栓系综合征;腰椎生理曲度正常7例,生理曲度变浅3例,生理曲度明显加深4例;患者均有脊髓位置下降;终丝横径正常者1例,余13例终丝横径均增粗;12例终丝内可见短T1长T2异常信号,2例终丝信号正常;14例患者中合并脊柱裂2例,双脊髓1例,脊髓空洞2例,骶管囊肿2例,中央管扩张1例,骶管裂1例;脊膜膨出2例,脂肪脊髓脊膜膨出12例。结论 MRI对诊断脊髓栓系综合征及手术方案的制定有一定价值。

Objective To study the MRI manifestations of tethered cord syndrome. Methods The MRI data were retrospectively analyzed in 14 cases of tethered cord syndrome. Results All these 14 cases were diagnosed by MRI. The physiological curvature of lumbar spine was normal in 7 cases, shallow in 3 cases and significantly enhanced in 4 cases. Spinal cord descended in all patients. Filum terminale diameter was normal in 1 case, and enlarged in the other 13 eases. The abnormal signals of short T1 and long T2 were seen in filum terminale in 12 cases and normal signals were seen in 2 cases. In 14 cases, 2 were complicated with spinal bifida, 1 double spinal cord, 2 syringomyelia, 2 sacral canal cyst, 1 enlargement of median cancal and 1 sacral crack. Meningoeele was found in 2 cases and lipomyelomeningocele was found in 12 cases. Conclusions MRI has a great value to the diagnosis and surgical treatment of tethered cord syndrome.
