颞部入路立体定向手术在老年重症脑桥出血治疗中的应用价值 被引量:6

摘要 目的探讨颞部入路立体定向手术在老年重症脑桥出血治疗中的应用价值。方法选择老年重症脑桥出血患者136例,根据治疗分为2组,手术组64例采取经颞部入路立体定向脑桥血肿引流手术治疗;对照组72例采取止血、脱水等治疗,2组有脑积水的患者给予脑室外引流,对比2组患者治疗效果。结果手术组病死率20.3%,对照组病死率58.3%,手术组治疗有效率53.1%,对照组有效率18.1%,2组比较差异均有统计学意义(P<0.05)。结论经颞部入路立体定向手术可改善重症老年脑桥出血患者预后。 Objective To study the application of temporal approach stereotactic surgery in elderly severe pontine hemorrhage patients.Methods One hundred and thirty-six elderly severe pontine hemorrhage patients were divided into operation group (n=64) and control group (n=72).Patients in operation group underwent temporal approach stereotactic operation for hematoma drainage and those in control group received hemostasis and dehydration treatment.Patients with hydrocephalus in both groups underwent external brain ventricle drasinage.The outcomes in the two groups were compared.Results The mortality was significantly lower while the effective rate was significantly higher in operation group than in control group (20.3% vs 58.3%,53.1% vs 18.1 %,P〈0.05).Conclusion Temporal approach stereotactic surgery can improve the outcomes in elderly severe pontine hemorrhage patients.
