摘要 随着我国临床疼痛科的建设和发展,如何构建疼痛医学的科学知识体系已经成为大家面临的一个迫在眉睫的问题。另外,如何把疼痛基础研究的现状和发展趋势介绍给临床医师也是基础研究工作者的重要任务。本文旨在抛砖引玉,系统地介绍慢性痛发生的外周和脊髓中枢敏化机制。此外,虽然我们对慢性痛及其精神共病(如焦虑、失眠、抑郁等)的脑机制了解甚少,但本文对可能涉及的脑网络基础也做了简要介绍。希望本文能够引起临床同行的共鸣,推动基础与临床研究的合作与发展。 With development of clinical pain medicine, how to build the scientific knowledge system of pain medicine is an urgent problem on one hand. On the other hand, how to translate the basic research breakthrough and trends to clinicians is also an important work of basic researchers. In the current review, we would like to introduce generally how chronic pain is developed and maintained at the peripheral and the spinal levels-- peripheral sensitization and central sensitization, in particular. Moreover, although less is known about the brain mechanisms of chronic pain and its mental comorbidities (such as anxiety, insomnia, and depression, etc.), the brain matrix (network) of pain is also introduced. Hopefully, this review can result in resonating effects on our clinical colleagues and promote collaborations between basic and clinical sides.
