帕金森病伴发精神障碍特点及其生活质量的影响

Characteristics of mental disorders in patients with Parkinson's disease and its influence on the quality of life

目的探讨帕金森病(PD)伴发精神障碍的特点及其对生活质量的影响。方法收集85例帕金森病患者资料,使用神经精神问卷(NPI量表)测定患者近1个月的精神症状,比较PD精神障碍组(PDP组)与PD无精神障碍组(NDNP组)生活质量的差异,并使用相关性分析研究神经精神症状的相关危险因素。结果 (1)54%的PD患者近1个月内出现过不同程度精神障碍。患者最常见的症状是睡眠障碍(36.47%)和抑郁(30.5%)。(2)PDP组与NDNP组日常生活活动能力和生活质量比较有统计学差异。(3)NPI评分与H-Y分期、病程、UPDRS-Ⅱ、UPDRS-Ⅲ、ADL呈正相关。结论 PD伴发精神障碍发病率高,以睡眠障碍及抑郁为主要表现,并严重影响患者的生活质量,PD伴发精神障碍与H-Y分期、病程、UPDRS-Ⅱ、UPDRS-Ⅲ、ADL相关,应对PD伴发精神障碍尽早识别、及时治疗,提高PD患者的生活质量。

Objective To investigate the features of mental disorders in patients Parkinson＇s disease（ PD） and its impacts on the quality of life. Methods 85 cases of PD patients were recruited forcollecting information. Using the neuropsychological questionnaire（ NPI scale） to value the neuropsychiatric symptom in the past month,to compare the differences of their quality of life between the PD patients with psychosis（ PDP group） and PD patients without psychosis（ NDNP group）,using correlation analysis to study the associated risk factors of neuropsychiatric symptoms. Results 54 percent of PD patients suffered from different kinds of mental disorders symptomswithin the past month,the most common symptoms were sleep disorders（ 36. 47%） and depression（ 30. 5%）. The differences of daily activities and quality of life between the two groups present statistically significant. NPI scores of PD patients with psychosis were positively correlated to H-Y stage,course of disease,UPDRS-Ⅱscores,UPDRS-Ⅲ scores,ADL scores. Conclusions Thepsychosis was indiscriminate in PD patients,which takes sleep disorders and depression as the main performanceand it affects their quality of lifeseriously. The occurrence of mental disorder was significantly correlated with the H-Y stage,course of disease,UPDRS-Ⅱscores,UPDRS-Ⅲ scores and ADL scores. Thus,it is very pivotal to recognize and treat mental disorder for PD patients to improve their quality of life.
