胸腰椎骨折手术治疗50例临床分析

Clinical Analysis of 50 Cases of Thoracic and Lumbar Fracture Operation

目的分析50例胸腰椎骨折患者的手术治疗效果。方法选择新沂市人民医院2012年1月至2012年10月收治的胸腰椎骨折患者50例,采用椎弓根螺钉系统内固定术进行治疗,分析不同时间段的手术时间与出血量的情况,观察患者治疗前后椎体压缩以及骨折脱位情况,比较患者治疗前后脊髓功能恢复情况。结果患者不同时间段手术出血量比较差异有统计学意义(P<0.05),2周内术中出血量显著低于其他各时间段的出血量(P<0.05);术后通过X线检查,患者椎体高度恢复情况良好,其中40例患者椎体高度完全恢复或基本恢复,骨折脱位椎体完全复位;患者术后脊髓功能显著优于术前(P<0.01)。结论椎弓根螺钉系统内固定术治疗胸腰椎骨折患者,能有效增强椎体强度,促进脊髓功能的恢复,效果满意。

Objective To analyze the therapeutic effect of 50 cases of thoracolumbar fracture operation. Methods A total of 50 cases with thoracolumbar fractures from Xinyi City People＇s Hospital during Jan. 2012 and Oct. 2012 were selected, pedicle screw fixation was used in the treatment, the operative time of dif- ferent time periods and the amount of bleeding were analyzed, the vertebral compression fractures and disloca- tions before and after treatment were observed, the spinal cord function recovery was compared. Results Surgical bleeding in patients with different time periods were statistically significantly different（ P 〈 0.05 ） , blood loss within 2 weeks was significantly lower than other time periods （ P 〈 0.05 ） ; X-ray examination after surgery found that vertebral height restoration was in good condition, including 40 cases of complete or basic recovery, and vertebral fracture dislocation completely reset;spinal cord function after surgery was significantly better than before surgery （ P 〈 0.01 ）. Conclusion Pedicle screw fixation in the treatment of thoraeolumbar fracture can effectively enhance the strength of the vertebral body, promote the recovery of the spinal cord function, with satisfactory effect.
