不同药物治疗84例妊娠高血压综合征的临床对比研究

Comparative Clinical Study of Different Types of Medications in the Treatment of 84 Cases of Preg-nancy-induced Hypertension

目的探讨不同药物治疗妊娠高血压综合征(PIH)的临床疗效。方法将浠水县人民医院2010年1月至2011年12月诊治的84例PIH患者依据随机数字表法分成两组:对照组(42例)采用单纯硫酸镁治疗,试验组(42例)采用硫酸镁+新利尿合剂(呋塞米、多巴胺、酚妥拉明)治疗。观察和比较两组的临床疗效、血压和24 h尿蛋白量的变化情况以及妊娠结局。结果试验验组的总有效率显著高于对照组(90.48%vs 73.81)(P<0.05);试验组治疗后患者的收缩压、舒张压、24 h尿蛋白量降幅均显著性大于对照组(P<0.05);试验组患者出现早产、胎盘早剥、剖宫产、产后出血、胎儿窘迫以及新生儿窒息等妊娠结局的概率显著低于对照组(P<0.05)。结论临床应用新利尿合剂辅助硫酸镁治疗PIH能够明显降低血压、改善妊娠预后。

Objective To investigate the clinical effects about different types of medications in the treatment of pregnancy-induced hypertension（PIH）.Methods A total of 84 cases of PIH admitted to Xishui People＇s Hospital from Jan.2010 to Dec.2011were randomly divided into two groups,control group（n=42） and treatment group（n=42）.The control group was given magnesium sulphate only,and the treatment group was also given new diuretic mixture（dopamine,phentolamine,furosemide）.The clinical efficacy,the blood pressure and 24 h urine protein,and the outcomes of pregnancy in the two groups were observed and compared.Results The total efficacy rate in the treatment group was significantly higher than the control group（90.48% vs 73.81）（P〈0.05）;the decline of systolic blood pressure,diastolic blood pressure,and 24 h urine protein in the treatment group after treatment were significantly higher than the control group（P〈0.05）;the incidence of preterm delivery,placental abruption,caesarean section,postpartum hemorrhage,fetal distress,and neonatal asphyxia in the treatment group were significantly lower than the control group（P〈0.05）.Conclusion The clinical application of new diuretic mixture combined with magnesium sulfate in the treatment of PIH can significantly reduce blood pressure and improve pregnancy outcome.
