闭合性阴茎海绵体损伤的诊治

Diagnosis and treatment of closed cavernous body fracture

目的：总结闭合性阴茎海绵体损伤的诊断和治疗体会.方法：2010年7月至2013年5月共收治9例闭合性阴茎海绵体损伤患者,诊断明确后均急诊行阴茎海绵体白膜修补术.结果：手术时间30～65min.术中出血量20～ 60mL.术后住院5～8d.随访3～12个月,无阴茎弯曲、阴茎硬结、勃起不坚、勃起疼痛、排尿困难、勃起功能障碍发生,性生活正常.结论：闭合性阴茎海绵体断裂患者伤后应尽快诊断,减少不必要的检查,以急诊手术治疗为好.使用可吸收缝线缝合海绵体白膜,减少术前等待时间,可降低术后纤维化的发生.

Objectives： To summarize the experience of diagnosis and treatment of closed cavernous body fracture. Method： From July 2011 to May 2013, a total of nine patients with closed cavernous body fracture were admitted in our hospital. Surgical repair of albuginea was conducted after diagnosis. Results： The operation duration was 30 - 65 minutes, the intraoperative blood loss was 20 - 60 ml, and the postoperative hospital stay was 5 - 8 days. They were followed up for 3 - 12 months. None of them had problems of phallocampsis, penile induration, weak erection, erectile pain, dysuria, or erectile dysfunction. They all had normal sexual life. Conclusion： Patients with closed cavernous body fracture should receive diagnosis and treatment as soon as possible. Emergency surgical repair is a fine choice. Using absorbable suture can reduce preoperative waiting time and reduce the incidence of postoperative fibrosis.
