症状性巨细胞病毒感染婴幼儿的听力损伤及治疗

SYMPTOMATIC CMV INFECTION OF INFANTS AND TREATMENT OF HEARING LOSS

目的研究症状性巨细胞病毒感染的婴幼儿听力损伤情况以及治疗措施。方法我院选择2010年7月—2013年7月诊治的110例症状性巨细胞病毒感染的患儿,对其进行ABR测试,比较治疗前后所选患儿的ABR值变化情况以及后期的语言发育情况。结果本文所选的110例患儿中,有74例患儿的双耳ABR阈值在正常范围,36例在异常范围,其中22例为单耳异常,14例为双耳异常,听力受损率约为总例数的32.73%(36/110),中或重度听力损伤的占16.36%(18/110)。经过治疗后,对ABR阈值再次检测,有14例阈值明显改善甚至恢复正常。第一次对46例患者进行电话随访,其中32例患儿听力语言发育良好,8例一般,4例发育较差,2例死亡。第二次对42例患儿进行随访,其中34例患儿与同龄儿的语言发育未见明显差异,可正常进入幼儿园及学前班学习,6例患儿与同龄儿相比有一定差距,2例死亡。结论婴幼儿受到症状性巨细胞病毒感染时听力损伤的几率相对较高,对其进行抗病毒治疗后能够程度不等的恢复听力损伤情况。

Objective To study the symptomatic CMV infection in infants hearing loss and treatment measures. Methods The hospital selected in July 2010 to July 2013 between the diagnosis and treat-ment of 110 patients with symptomatic CMV infection in children, its ABR test , before and after treatment in children with ABR selected value changes as well as the late language development situa-tion. Results Totally 110 patients were chosen for this article, there are 74 cases of children with bin-aural ABR thresholds in the normal range, 36 cases in the abnormal range, including 22 cases of single - ear abnormalities, 14 cases of abnormal ears, hearing＇impaired rate is about 32.73% of total number of cases （36/110） , moderate or severe hearing loss accounted for 16. 36% （18/110）. After treatment of the ABR threshold is detected again, there are 14 cases of threshold significantly improve or even back to normal. The first 46 cases of patients with telephone follow-up, including 32 cases of lan-guage development in children with hearing good, 8 cases in general, poorly developed four cases, 2 deaths. The second pair of 42 infants was followed up, 34 cases of children with language developme-ntal age children showed no differences, normally enter kindergarten and preschool learning, 6 infants compared with their peers children have a certain gap, two cases deaths. Conclusion Infant suffer symptoms of CMV infection is relatively high probability of hearing loss, its anti-viral treatment to varying degrees of hearing loss recovery.
