放疗途径对肺V5、V20、V30及放射性食管炎发生率的影响

THE EFFECT OF RADIOTHERAPY APPROACH ON LUNG V5,V20,V30 AND RADIO OESOPHAGITIS INCIDENCE

目的探讨放疗途径对肺V5、V20、V30及放射性食管炎发生率的影响。方法选取我院2010年1月—2012年1月收治的100例食管癌患者,将其作为临床研究对象,随机分为对照组和观察组,每组50例。所有食管癌患者直接进行放疗,对照组患者采取三维适形放射治疗,观察组患者采取适形强调放疗。观察二组患者靶区的放疗剂量及肺V5、V20、V30等参数情况以及放射性食管炎的发生情况。结果二组患者靶区剂量Dmax、Dmin、Dmean比较差异无统计学意义(P>0.05);而观察组患者CI为(0.728±0.08),明显高于对照组;对照组V5优于观察组,而观察组V20、V30则优于对照组;观察组放射性食管炎的发生率为20.0%,明显低于对照组52.0%,差异均具有统计学意义(P<0.05)。结论适形强调放疗对食管癌的治疗效果好,靶区剂量高,强调放疗可以有效减少肺V20、V30,对肺起到保护作用,且放疗后放射性食管炎的发生率低,值得在临床上广泛应用。

Objective To explore the clinic effect of radiotherapy on lung V5, V20, V30 and radiation esophagitis rate. Methods In author＇s hospital from January 2010 to January 2012, 100 patients with esophageal cancer were treated, as a clinical study were randomly divided into control group and obser-vation group, 50 cases in each group. All patients with esophageal cancer underwent radiotherapy di-rectly, control group patients received three- dimensional conformal radiation therapy, observation group was taken conformal radiotherapy emphasized. Targets were observed radiation dose and lung VS, V20, V30 and other parameters as well as the occurrence of esophagitis. Results The target dose Dmax, Dmin, Dmean was no significant difference （P〉0.05） between two groups patients; while the observation group were CI = （0. 728 ± 0.08）, significantly higher; observation group than the control group V5, and observation group, V20, V30 were better than the control group; observation group esophagitis occurred was 20.0%, significantly lower than the control group（ 52.0%）, the differences were statistically significant （P〈0.05）. Conclusion Conformal radiotherapy for esophageal empha-size the therapeutic effect of good, high target dose, stressing radiotherapy can effectively reduce lung V20, V30, play a protective effect on the lungs, and after radiotherapy low incidence of esophagitis, it is in clinical widely applied.
