早期介入治疗在颅内动脉瘤破裂治疗中的临床价值

CLINICAL VALUE OF EARLY INTERVENTION IN THE TREATMENT OF RUPTURED INTRACRANIAL ANEURYSM

目的探讨早期介入治疗在颅内动脉瘤破裂中的临床应用效果。方法选择我院收治的60例颅内动脉瘤破裂患者为研究对象,根据血管内治疗距发病时间分为早期手术组和晚期手术组,二组均行介入治疗。比较二组脑血管造影栓塞程度,以格拉斯哥预后评分评价预后情况,以神经功能缺损程度评分评价神经功能改善情况。结果早期组患者血管内治疗后栓塞程度显著高于晚期治疗组患者完全栓塞程度,差异有统计学意义(P<0.05)。早期介入治疗患者术后肢体活动障碍发生率显著低于晚期介入治疗组,差异有统计学意义(P<0.05)。早期介入组预后良好率显著高于对照组,差异有统计学意义(P<0.05)。早期介入组治疗后总有效率显著高于晚期介入组,差异有统计学意义(P<0.05)。结论在掌握好手术时机的情况下,早期明确诊断,迅速给予病因治疗,可有效提高颅内动脉瘤破裂的治愈率,降低病死率及致残率,提高患者的生存质量。

Objective To study the clinical application effect of early intervention in the treatment of intracranial aneurysms. Methods All 60 cases of ruptured intracranial aneurysm patients were select-ed as the research subjects, were divided into the early surgery group and surgery group according to the endovascular treatment of late onset time, all subjects were performed in interventional therapy. The cerebral angiogram embglization degree was compared in two groups, evaluation of prognosis was performed by Glasgow prognostic score, and improvement of nerve function was also evaluated on de-gree of nerve function defect score. Results The group of patients with early endovascular emboliza-tion degree after the late treatment group was significantly higher than patients treated completely em-bolization degree, the difference was statistically significant （P〈0.05）. Early intervention treatment of patients with postoperative activity obstacle incidence was significantly lower than the late interven-tion group, the difference was statistically significant （P〈0.05）. Early intervention group good prog-nosis rate was significantly higher than the control group, the difference was statistically significant （P 〈0.05）. After early intervention treatment group total effective rate was significantly higher than the late intervention group, the difference was statistically significant （P〈0.05）. Conclusion In the case of mastering operation time, early diagnosis and prompt etiological treatment can effectively improve the cure rate of intracranial aneurysm rupture, reduce the mortality and morbidity, improve the life quality of patients.
