内镜窄带成像结合染色技术在早期胃癌筛查中的临床价值

Clinical Value of Narrow-band Imaging Combined with Chromoendoscopy in Early Gastric Cancer Screening

目的:回顾性分析内镜窄带成像技术(NBI)结合染色技术在早期胃癌筛查中的临床价值。方法:对2007年1月-2008年4月(普通白光内镜组)和2009年1月-2012年5月(NBI结合染色技术组)在我院消化内镜中心接受胃镜检查的病例进行回顾性分析,分别记录胃镜镜下诊断及病理诊断的结果,以病理诊断为金标准,采用Kappa一致性检验评价2组病例镜下诊断与病理诊断的一致性。结果:普通白光内镜组与NBI结合染色技术组早期胃癌的检出率分别为4.51%和10.38%,Kappa值分别为0.683和0.837(均P<0.01)。结论:NBI结合染色技术可提高早期胃癌镜下诊断的准确性。

Objective：To retrospective analyze the clinical value of narrowband imaging （NBI） combined with chro- moendoscopy in early gastric cancer screening. Methods： Cases from January 2007 to April 2008 （the White light endos- copy case） and January 2009 to May 2012 （the NBI combined with Chromoendoscopy case） were retrospectively ana- lyzed,recorded the results of endoscopic diagnosis and pathological diagnosis, Pathological diagnosis was used as the gold standard. The concordance between endoscopic diagnosis and pathological diagnosis was evaluated through the a- greement （Kappa） test. Results： The Kappa values of the White light endoscopy case and the NBI combined with Chro- moendoscopy case were 0. 683 and 0. 837 （all P〈0. 01）. Conclusion： NBI combined with Chromoendoscopy may im- prove the diagnostic accuracy of early gastric cancer.
