肿瘤标志物TSGF与CA19-9联合检测在良、恶性腹水鉴别诊断中的临床意义

Clinical significance analysis of tumor markers of tumor specific growth factor,CA19-9 combined detection in benign and malignant ascites' differential diagnosis

目的研究肿瘤标志物肿瘤特异生长因子(Tumor Supplied Group of Factors,TSGF)与CA19-9联合检测在良、恶性腹水鉴别诊断中的临床意义。方法选取2010年3月—2013年3月我院收治的112例腹水患者,空腹抽取晨间静脉血后,采用免疫法和化学发光法测定TSGF、CA19-9水平,并对血清和腹水TSGF、CA19-9水平及诊断结果进行比较分析。结果恶性腹水组血清、腹水TSGF水平和腹水/血清比值均显著高于良性腹水组;恶性腹水组血清、腹水CA19-9水平和腹水/血清比值均显著高于良性腹水组,差异均有统计学意义(均P<0.05)。联合检测方案的敏感性、准确性最佳,其中腹水敏感度为88.9%,准确度为90.2%;血清敏感度为74.6%,准确度为81.2%。结论 TSGF与CA19-9联合检测对鉴别良、恶性腹水具有重要的诊断价值,值得临床推荐应用。

Objective To study clinical significance analysis of tumor markers of tumor specific growth factor （TS GF）,CA19-9 combined detection in benign and malignant ascites＇ differential diagnosis.Methods From March 2010 to March 2013,the hospital treated a total of 112 cases of patients with ascites.Took the morning fasting venous blood used immunoassay and chemiluminescence to test TSGF and CA19-9 levels,compared with serum and ascites＇ TSGF and CA19-9 levels,and result of serum,ascites TSGF and CA19-9 diagnosis.Results The malignant ascites group＇s ascites and serum TSGF level,ascites/serum ratio were significantly higher than benign ascites group;the malignant ascites group＇s ascites and serum CA19-9 level,ascites/serum ratio were significantly higher than benign ascites group.Differences were statistically significant （all P＜0.05）.Combined detection＇s sensitivity and accuracy was the best,in which the serum＇s sensitivity was 88.9％ and accuracy was 86.6％.Ascites sensitivity was 74.6 ％ and accuracy was 81.2％.Conclusion TSGF and CA19-9 combine detection for differentiating benign and malignant ascites has great value.Worthy of recommended for clinical applications.
