联合检测血清IMA、hs-CRP与cTnI在急性冠状动脉综合征(ACS)的诊断价值 被引量:20

摘要 目的探讨缺血修饰白蛋白(IMA)、超敏C反应蛋白(hs-CRP)和心肌肌钙蛋白I(cTnI)在急性冠状动脉综合征(ACS)早期诊断中的价值。方法选取75例冠心病人:稳定性型心绞痛(SAP)25例,不稳定型心绞痛(UAP)28例,急性心肌梗死(AMI)22例,于入院后即刻取血测定其血清IMA、hs-CRP及cTnI的变化,并与25例健康者(对照组)进行比较。结果与对照组和SAP组比较AMI组三项指标水平有显著性差异(P<0.01),UAP组的IMA与hsCRP水平有显著性差异(P<0.01);与UAP组比较AMI组的hs-CRP和cTnI水平差异有显著性意义(P<0.01),而IMA水平无统计学意义(P>0.05);SAP组与对照组比较三项指标水平无统计学意义(P>0.05)。结论 IMA可作为急性心肌缺血的灵敏诊断指标;IMA、hs-CRP和cTnI联合检测可评估ACS患者心肌损伤程度,具有重要的诊断、指导治疗价值。 Objective To investigate the early diagnosis value of serum ischemia modified albumin(IMA) 、high sensitive C-reactive protein(hsCRP)and cardiac troponin Ⅰ (cTnⅠ) in patients with acute coronary syndrome (ACS).Methods The 70 patients with coronary artery disease were divided into three groups:the stable angina pectoris (SAP) 25 cases,the unstable angina pectoris (UAP) 28 cases,the acute myocardial infarction (AMI) 22 cases,and select the healthy control group 25 subjects.The levels of four groups' IMA、hs-CRP and cTnⅠ were detected and Compared.Results The levels of three indicators of the AMI group were significantly increased (P<0.01)and the levels of IMA、hs -CRP of the UAP group were significantly higher than those the control group and the SAP group (P<0.01);Compared with the UAP group,the levels of hs-CRP and cTnⅠ were significantly higher in AMI group(P<0.01),while the level of IMA there was no significantly difference(P>0.05) ;The levels of three indicators were no significantly difference between SA group and control group (P>0.05).Conclusion IMA can be a sensitive diagnosis indicator of acute myocardial ischemia.Joint detection the levels of serum IMA、hsCRP、cTnⅠ can be used to estimate the extent of myocardial injury,and play an important role in the diagnosis and treatment.
