摘要
目的 :对社区高血压患者进行群组干预管理,分析评价实施6个月后患者的生命质量变化情况。方法:按照随机对照试验研究设计,将400名参加该研究的高血压病人随机分为200名干预组和200名对照组。干预组接受高血压群组干预管理,对照组接受常规的高血压三级管理。6个月后比较2组病人在项目实施前后的生命质量量表(SF-36)评分及部分生物检测指标的变化情况。结果 :干预6个月后,除生理机能和躯体疼痛外,干预组患者的生理职能、情感职能、精力、精神健康、社会功能以及一般健康状况等SF-36各维度变化值均明显高于对照组(P<0.05)。6个月后干预组患者的收缩压和舒张压分别比对照组多下降4.84 mmHg和2.07 mmHg(P均<0.05)。体质指数、腰臀比的变化无显著差异。结论 :与常规的三级管理相比较,社区高血压群组干预管理能更好地提高患者的生命质量,改善患者的血压控制。
Objective: To practice hypertension group-interference-management in community and to analyze the change of life quality of patients after 6-month-implementation. Methods : The study conducted a randomized controlled triM, in which 400 patients with hypertension were randomized into two groups. Intervention group( n=200 ) received the group-interference-management and control group ( n=200 ) received routine hypertension management services. Before the study and 6 months after the beginning of experiment, questionnaire survey was conducted to investigate the changes in life quality scale( SF-36 ) and changes of biological indexes, respectievly. Results: Except physical function ( PF ) and bodily pain ( BP ), the subjects in the intervention group showed greater improvement in role-physical ( RP ), role-emotional ( RE ), vitality ( VT ), mental health ( MH ), social functioning ( SF ) and general health ( GH ) in comparison with the control group ( P〈0.05 ). The value of systolic blood pressure and diastolic blood pressure in the intervention group decreased by 4.84 mmHg and 2.07 mmHg, more than those in the control group ( P〈0.05 ). Changes of body mass index and waist-to-hip ratio showed no statistical significance. Conclusion: Compared with the routine hypertension management services, the community-based hypertension group-interference- management model is more effective in the improvement of life quality of patients and blood pressure control.
