摘要
目的:系统评价带状疱疹并假性肠道梗阻相关研究文献,以减少带状疱疹致假性肠梗阻的误诊误治,提高诊断及疗效。方法:计算机检索PubMed、Embase、CNKI、万方、VIP及Cochrane等数据库。由两名研究者按照纳入排除标准筛选文献,提取资料及表格并评价质量。结果:(1)老年男性为高发人群;免疫力低下为主要诱因,国内诱因不详;(2)皮肤及腹部症状为主要症状,腹部X线平片为确诊手段;(3)国内外以一般治疗、胃肠减压及抗病毒治疗为主,国内外治疗效果相近,无并发症报道。结论:免疫力低下老年男性为高发人群;皮肤及腹部症状为主要症状;国内外各研究一致性认为腹部X线平片为确诊手段,一般治疗、胃肠减压及抗病毒为主要治疗手段,疗效相近;纳入证据级别低,该病临床诊断治疗过程中,尚需结合临床医生经验及患者价值观。
Objective:To systematically review all the relevant literatures,to reduce the misdiagnosis and mistreatment of Herpes zoster induced-intestinal pseudo-obstructions and improve the diagnosis and treatment.Method:Relevant articles were searched potentially in the databases PubMed,Embase,CNKI,WanFang,VIP and the Cochrane library through computer.Two reviewers selected all studies according to the inclusion and exclusion standard,and extract information and forms,and evaluate quality. Result:(1)The high-risk groups were the older men. Diseases which could reduce immunity were mainly incentives for it. Most articles did not descript the detailed incentives in china.(2)Skin and abdominal symptoms were the main symptoms,abdominal X-ray plain film was definitive diagnostic method.(3)In all studies,supportive treatment,gastrointestinal decompression and anti-viral treatment were mainly therapies.Treatment effect was similar in china and foreign. There was not report of special complications.Conclusion:The high-risk groups were the older men with low immunity;Skin and abdominal symptoms were the main symptoms,at both home and abroad abdominal X-ray plain film was definitive diagnostic method,supportive treatment,gastrointestinal decompression and anti-viral treatment were mainly therapies. And treatment effect was similar in china and foreign. According to the low level of evidence,it is combined with clinical experience and values in patients with a doctor in the process of the disease clinical diagnosis and treatment.
