布地奈德混悬液异丙托溴铵和硫酸沙丁胺醇溶液联合雾化吸入治疗AECOPD临床观察

Observation on the Effect of Budesonide Suspension Inhalation Ipratropium Bromide and Salbutamol Sulfate Solution Combined Atomization Inhalation in Treatment of AECOPD

目的:探讨布地奈德混悬液联合吸入用异丙托溴铵和硫酸沙丁胺醇溶液(复方异丙托溴胺溶液)雾化吸入治疗急性加重期慢性阻塞性肺部疾病(AECOPD)的临床价值。方法:选取我院2009年1月至2011年12月我院收治的AECOPD患者110例,随机分为两组,分别采用吸入用异丙托溴铵和硫酸沙丁胺醇溶液雾化吸入和布地奈德混悬液联合雾化吸入治疗。结果:研究组患者咳嗽症状消失时间相比对照组无显著性差异(P>0.05),喘憋、湿啰音、哮鸣音消失时间与住院时间相比对照组均明显减少(P<0.05)。两组患者治疗前PEF、PaO2、PaCO2、呼吸困难评分比较无明显差异(P>0.05),治疗后研究组组患者PEF、PaO2、PaCO2、呼吸困难评分均较治疗前明显改善(P<0.05),治疗后研究组PEF、PaO2、PaCO2、呼吸困难评分相比对照组有明显改善(P<0.05)。结论:吸入用异丙托溴铵和硫酸沙丁胺醇溶液联合布地奈德混悬液相比单用复方异丙托溴胺溶液雾化吸入治疗AECOPD能够有效缩短临床症状与体征消失时间、住院时间,明显改善呼吸功能情况。

Objective：To investigate the clinical value of budesonide suspension combined inhalation of supporting bromide and salbutamol sulfate solution with isopropyl （ compound ipratropium bromide solution ） inhalation in the treatment of acute exacerbation of chronic obstructive pulmonary disease （ AECOPD ） . Method：110 patients with AECOPD in our hospital from 2009 January to 2011 December were selected , they were randomly divided into two groups , They were respectively treated by inhalation of ipratropium bro-mide and salbutamol sulfate solution and atomization inhalation of budesonide suspension combined inhalation therapy .Result：The cough symptoms disappeared time of the research group had no significant difference compared with control group （P ＆gt;0.05）, while, wheezing, wet rales, wheeze disappeared time of the re-search group significantly reduced （P＆lt;0.05）.Before treatment, PEF, PaO2, PaCO2, dyspnea scores of two groups of patients showed in significant difference （P＆gt;0.05）, after treatment PEF, PaO2 group, PaCO2, dyspnea scores of research group were significantly improved （P＆lt;0.05）, PEF, PaO2, PaCO2, dyspnea score of research group were obviously improved comparing with the control group （P＆lt;0.05）.Conclusion：Inhalation of ipratropium bromide and salbutamol sulfate solution combined with budesonide suspension com -paring with the single compound ipratropium bromide solution for inhalation in the treatment of AECOPD can effectively shorten the clinical symptoms and signs disappeared time , hospitalization time , and improve re-spiratory function .
