摘要
目的观察中西医结合治疗慢性肺原性心脏病(简称肺心病)急性加重期并消化道症状的临床疗效及安全性。方法将120例肺心病急性加重期并消化道症状患者随机分为治疗组和对照组各60例,对照组给予西医基础治疗,并给予多潘立酮片10mg,每日3次。治疗组在对照组治疗的基础上辨证给予中药治疗,每日1剂,两组均治疗7天后评价临床疗效。两组于治疗前后进行中医症状评分,并判定临床疗效及不良反应。结果治疗组临床疗效总有效率为91.67%,对照组为71.67%,治疗组明显高于对照组(P<0.01)。治疗组治疗后各症状积分明显减少(P<0.01);治疗组治疗后胃脘疼痛、饮食减少、口苦口干、恶心呕吐、胃中嘈杂症状积分明显少于对照组(P<0.01),治疗组治疗后中医证候积分明显少于对照组(P<0.01)。治疗组未见明显不良反应。结论中西医结合治疗肺心病急性加重期并消化道症状疗效优于单纯西药治疗,能显著改善消化道症状且安全可靠。
Objective To observe the efficacy and safety of integrative medicine for acute exacerbation of chronic pulmonary heart disease (CPHD) complicated by gastrointestinal symptoms. Methods Totally 120 cases of acute exacerbation of CPHD complicated by gastrointestinal symptoms were randomized into treatment group and control group, with 60 in each. The control group was given basic western medicine treatment and 10mg domperidone tablets three times daily. The treatment group was added with TCM formula based on syndrome identification. The therapeu- tic effect was evaluated after 7-day treatment. The scores of TCM syndrome were observed before and after treatment. Side effects and therapeutic effect were evaluated. Results The total effective rate of the treatment group and con- trol group was 91.67% and 71.67% respectively, with significant difference between groups (P 〈0.01 ). The scores of all symptoms were significantly decreased in the treatment group ( P 〈 0. 01 ). After treatment, the scores of stomach pain, poor appetite, bitter mouth, dry mouth, nausea, vomiting and gastric upset in the treatment group were signifi- cantly lower than those in the control group (P 〈 0. 01 ). The total score of TCM patterns was significantly decreased after treatment in the treatement group (P 〈 0. 01 ). Conclusion The integrative medicine is effective and safe for acute exacerbation of CPHD complicated by gastrointestinal symptoms and it can relieve gastrointestinal symptoms.
