摘要
目的采用汉语成语作为N400刺激物,探讨神经衰弱与抑郁症患者事件相关点位N400的关系。方法选择2011年3月—2013年3月在本院心理门诊和住院的神经衰弱和抑郁症患者,并选取40例正常人作为对照组。采用汉语成语作为N400刺激物,观察受试者的N400的测量。结果 MMSE评分B组、A组、C组呈上升趋势,且两两之间比较均有显著差异(P均<0.01);HAMD-17总分B组、A组、C组呈逐渐下降趋势,且两两之间比较也均有显著差异(P均<0.01);N400潜伏期B组、A组、C组呈逐渐下降趋势,且两两之间比较均有显著差异(P均<0.05);N400波幅C组、A组、B组呈逐渐下降趋势,且A组与C组、B组与C组间存在显著差异(P均<0.01);抑郁症患者组的N400潜伏期延长与HAMD-17评分呈正相关关系(r=0.321,P=0.026)。结论相较于正常人,神经衰弱与抑郁症患者的思维减慢,对汉语成语的理解、抽象概括能力降低,且抑郁症患者表现得更为突出;抑郁症患者Cz脑区的N400潜伏期与HAMD-17评分是呈正相关的。
Objective To explore the application of Chinese idom-N400 in the detection of event-related potential in patients with neurasthenia and depression. Methods Patients with neurasthenia and depression from March 2011 to March 2013 were collected and 40 healthy people were designed as control group. The Chinese idom-N400 was used as stimulant to observe the N400quantity of patients.Results MMSE score increased gradually in group B, group A and group C,and there were significant differences between every two groups（P 0.01）. The total score of HAMD-17 decreased gradually in group B, group A and group C, and there were significant differences between every two groups（P 0.01）.Latency stage of N400 reduced gradually in group B,group A and group C, and there were significant differences between every two groups（P 0.01）.Amplitude of N400 reduced gradually in group C, group A and group B, and there were significant differences between group A and C as well as group B and C（P 0.01）.There was a positive correlation between the prolong of latency stage of N400 and HAMD-17 score in patients with depression（r= 0.321,P= 0.026）. Conclusion Compared with the normal people, patients with neurasthenia and depression show a declinesof cogitation,understanding,abstract and recapitulation abilities,especially for patients with depression. There is a positive correlation between latency stage of N400and HAMD-17 score in CZ cerebral area of patients with depression.
