整体护理干预对老年阵发性房颤患者餐后低血压的影响

Influence of holistic nursing care intervention on postprandial hypotension in elderly patients with paroxysmal atrial fibrillation

目的探讨整体护理(HNC)干预对老年阵发性房颤(PAF)患者餐后低血压(PPH)发生的的影响。方法选择2011—2012年在湖北医药学院附属太和医院住院的196例老年冠心病PAF伴PPH患者,随机分为整体护理组98例与传统护理组98例;分别给予整体护理和传统护理干预;分析、比较2组患者状态焦虑水平,住院期间PAF、PPH以及两者伴发的次数等差异。结果①整体护理组状态焦虑水平住院第3天(52.47±5.31)分、出院当天(39.79±4.16)分与传统护理组(73.38±6.94)分、(57.52±5.58)分分别比较,差异明显(P<0.01)。②整体护理组PAF、PPH以及两者伴发的发生次数分别为(3.55±0.37)、(5.29±0.49)及(2.54±0.32)次,与传统护理组的(5.23±0.49)、(7.56±0.79)及(3.51±0.37)次比较,有明显减少(P<0.01)。③整体护理组患者的满意度、护患沟通率、健康教育覆盖率及住院时间,与传统护理组比较,前三者有明显提高、后者有明显缩短(P<0.01)。结论整体护理可以降低老年PAF患者状态焦虑水平和PPH发生率。

[Objective]To explore the influence of holistic nursing care （ HNC） intervention on postprandial hypotension （PPH） in elderly patients with paroxysmal atrial fibrillation （PAF）. [Methods] 196 elderly patients of coronary heart disease-induced PAF combined with PPH that were admitted to our hospital from 2011 to 2012 were randomly divided into holistic nursing group and tra？ditional nursing group, 98 cases in each group. They were given holistic nursing intervention and traditional nursing intervention re？spectively. The levels of state anxiety of patients in two groups were compared for analysis. The differences of the frequence of PAF, PPH and combination of the two diseases in the groups were compared during their hospitalization. [ Results]①The levels of state anxiety were （52.47 ±5.31） on the third day of admission and （ 39. 79 ±4, 16） on the day of discharge in holistic nursing group, and were （73. 38 ±6.94） and （57.52 ±5.58） in traditional nursing group respectively. The difference was statistically significant （p 〈 0 . 01）＇ ②The frequence of PAF,PPH and combination of the two diseases were （3 ？ 55 ± 0, 37）,（5. 29 ± 0. 49） and （2. 54 ±0.32） respectively in holistic nursing group. All of them were significantly reduced compared with those in the tradi？tional nursing group （5.23 ±0.49） , （7. 56 ± 0. 79 ） , （4. 51 ＋ 0. 37 ） （ P 〈 0. 01）.③ Compared with the traditional nursing group,the satisfaction degree of patients,communication rate of nurses and patients,coverage rate of health education in holistic nursing group obviously improved and hospital stays significantly reduced （P 〈0. 01）. [ Conclusion] Holistic nursing care can de？crease the level of state anxiety and the incidence of postprandial hypotension of elderly patients with paroxysmal atrial fibrillation.
