摘要 评价结核感染T细胞酶联免疫斑点试验(T-SPOT)在儿童结核性疾病中的诊断价值.对2011年7月至2012年6月苏州大学附属儿童医院收治的45例疑诊或待排结核患儿进行T-SPOT检测,并与结核菌素纯蛋白衍生物(PPD)皮肤试验、血结核抗体测定、抗酸杆菌涂片及结核菌DNA-PCR进行比较.T-SPOT的敏感度、特异度分别为92%、95%,明显高于PPD皮肤试验(76%、75%),差异有统计学意义(P<0.01),两者诊断符合率为18/19.T-SPOT的阳性预测值为96%,阴性预测值为90%.T-SPOT是诊断儿童结核性疾病快速、敏感、特异的方法,在结核性疾病早期诊断中有重要价值. To evaluate the diagnostic value of interferon-gamma release assay T-SPOT for childhood tuberculosis. A total of 45 children suspected of tuberculosis at Affiliated Children Hospital, Soochow University were enrolled from July 2011 to June 2012. T-SPOT test was performed and its results were compared with those of PPD test, tuberculosis antibody test, acid-fast bacilli stain and tuberculosis polymerase chain reaction. The sensitivity and specificity of T-SPOT were 92% and 95% respectively. And the values were higher than PPD test (76%, 75%, P 〈 0. 01 ). The overall concordance rate between two tests was 18/19. Positive and negative predictive values of T-SPOT were 96% and 90% respectively. Thus T-SPOT test is rapid, sensitive and specific for diagnosing tuberculosis. It can be applied for an early diagnosis of tuberculosis in children.
