应用防噪声弹性耳塞对非全麻手术患者焦虑状态的影响研究

Influences of noise-proof elastic earplugs application on anxious state of patients subject to non-general anesthesia operations

目的 探析应用防噪弹性耳塞对非全麻手术患者焦虑状态以及生命体征的影响.方法 选取浙江省瑞安市人民医院2013年5月～2013年10月收治的非全麻下行剖宫产产妇200例为研究对象,采用前瞻性研究方法,将其按照随机数字表法分为两组,对照组100例仅采用临床常规护理,研究组100例应用防噪声弹性耳塞等人性化护理,评估两组患者在术前、术中、术后的焦虑状态,并记录术前、手术开始后每隔10 min的血压、脉搏等生命体征,同时采用本院自制的护理满意调查表调查分析产妇对护理的满意度.结果 ①两组产妇术前SAS评分比较,差异无统计学意义（P＞0.05）;对照组产妇术中和术后SAS评分[（44.65±12.68）、（43.38±9.46）分]高于术前[（40.38±10.86）分],差异有统计学意义（P＜0.05）;研究组产妇术中和术后SAS评分[（36.56±10.26）、（30.52±8.54）分]低于术前[（40.86±10.65）分],差异有统计学意义（P＜0.05）;研究组产妇术中、术后SAS评分明显低于对照组（P＜0.05）;②两组患者术前血压、脉搏比较,差异无统计学意义（P＞0.05）,研究组产妇手术开始10 min血压较术前稍有下降（P＜0.05）,20 min后与术前相比,差异无统计学意义（P＞0.05）,手术前后脉搏基本无变化（P＞0.05）;对照组产妇手术开始10 min血压、脉搏均有所增加,20 min后血压和脉搏较术前均明显增加,差异有统计学意义（P＜0.05）;手术开始后10、20 min研究组收缩压、脉搏明显低于对照组（P＜0.05）;③研究组产妇护理满意度为（100％）明显高于对照组（84％）,差异有统计学意义（P＜0.05）.结论 非全麻手术患者手术室护理过程中采用防噪声弹性耳塞等人性化护理措施,能有效缓解产妇的焦虑状态,平稳患者生命体征,保障手术安全,同时进一步提高护理质量,值得在临床护理过程中广泛地推广和应用.

Objective To explore influences of noise-proof elastic earplug application on anxiety state and vital signs of patients subject to general anesthesia operations Methods 200 puerperaes with cesarean deliveries om March 2012 to May 2013 in People＇s Hospital of Rui＇an City in Zhejiang Province were selected as research subjects,and they were divided into two groups according to the random number table method,100 cases of puerperae were only subject to clinical routine nursing care in the control group,while 100 cases of puerperae in research group were subject to noise-proof elastic earplugs humanized nursing,the preoperative,intraoperative and postoperative anxiety states of puerperae in two groups were assessed,preoperative and intraoperative ital signs of patients （including the blood pressure and pulse） every ten minutes were recorded,and homemade nursing satisfaction questionnaire was applied to investigate and analyze nursing satisfaction.Results ①Comparative differences of preoperative SAS scores of puerperae in two groups,the difference was statistically significant （P 〈 0.05）; intraoperative and postoperative SAS scores of puerperae in the control group [（44.65±12.68）,（43.38±9.46） scores] were higher than that preoperative scores [（40.38±10.86） scores],the difference was statistically significant （P 〈 0.05）; intraoperative and postoperative SAS scores of puerperae in the research group [（36.56±10.26）,（30.52±8.54） scores] were lower than preoperative scores [（40.86±10.65） scores],the difference was statistically significant （P 〈 0.05）; postoperative SAS scores of puerperae in the research group were significantly lower than that in the control group （P 〈0.05）.②Comparative differences of reoperative blood pressure and pulse of puerperae were not signific ant （P 〉 0.05）,the blood pressure of puerperae slightly decreased （P 〈 0.05） since the commencement of operations,postoperative blood pressure of puerperae were basically identical to that in 10 minutes since commencement of operatios （P 〉 0.05）,the pulse after operation was not change basically compared with before operation （P 〉 0.05）.The pulse and blood pressure of puerperae in the control group increased slightly in 10 min upon the commencement of operations,blood pressure and pulse of puerperae significantly increased in 20 min after the commencement of operations （P 〈 0.05）.The systolic blood pressure and pulse of puerperae were significantly lower than that in the control group in 10 minutes and 20 minutes after operations compared with preoperative （P 〈 0.05）.③The nursing satisfaction of puerperae in the research group was 100%,which was significantly higher than that in the control group （84%）,the difference was statistically significant （P 〈 0.05）.Conclusion The adoption of humanized nursing measures including noise-proof elastic earplugs in the process of nursing patients subject to non-general anesthesia operations in operating rooms can effectively alleviate the anxiety state of puerperae,stablize vital signs of patients,ensure the safety of operation,and further improve the quality of nursing,being worth of widely popularizing and applying in the process of clinical nursing.
