N-端脑钠肽前体在肺心病合并右心衰竭患者诊断及疗效中的价值 被引量:2

摘要 目的研究N-端B型脑钠肽前体(N-terminal por-brain natriuetie peptide,NT-proBNP)在肺心病(pulmonary heart disease,PHD)合并右心衰竭患者诊断及疗效中的价值。方法随机选取PHD合并右心衰竭患者50例作为实验组,PHD未合并右心衰竭患者30例为对照组。采用电化学发光双抗体夹心法测定实验组、对照组入院后及实验组心衰纠正后的血浆N-端脑钠肽前体的质量浓度和血气分析指标。结果实验组N-端脑钠肽前体值明显高于对照组(P<0.001)。实验组心衰纠正后N-端脑钠肽前体值明显下降,与治疗前比较P<0.001,差异有统计学意义。NT-proBNP与PaO2存在直线相关关系(r=0.953,P<0.01),与PaCO2无关。结论血浆N-端脑钠肽前体能较好地反映PHD患者的心功能状态,对于PHD合并右心衰竭患者的诊断及疗效有一定的临床价值。 Objective To study the value of N-terminal pro-brain natriuretic peptide in diagnosis and efficacy of pulmonary heart disease with right heart failure. Methods Fifty patients of pulmonary heart disease with right heart failure were random setected as experimental group.Thirty patients with pulmonary heart disease without right heart failure were served as control group.The plasma mass concentrations of NT-proBNP of experiwental group, control group and experimental group's heart failure after correction were detected by electro chemiluminescence immunoassay. Results The plasma mass concentrations of experimental group's is significantly higher than control group's (P〈0.001). After the experimental group's heart failure is corrected,the plasma mass concentrations of NT-proBNP is significantly decreased, compared with before treatment P〈0.001, The difference was statistically significant. There was linear regression relationship between NT-proBNP and PaO2 (r=0.953, P〈0.01), without relationship with PaCO2. Conclusion plasma NT-proBNP can he used to evaluate the heart function of patients with pulmonary heart disease, and are useful tools for diagnosis and efficacy of pulmonary heart disease with right heart failure.
